We are hiring a Shared Living Planning Coordinator to join our Shared Living team based in Lewiston, ME. This is a full time position with a Monday to Friday day time schedule. Shared Living is a housing and support service for people with intellectual and developmental disabilities (IDD) or autism, and our Shared Living team provides support to Shared Living Providers who have decided to open their homes to caring for a participant, from the matching and placement process through to ongoing mentorship.
JOB DESCRIPTION SUMMARY: This position is responsible for assisting with tasks associated with the Person-centered planning process, as well as the initial set-up of the Shared Living arrangements. This position will report to the Shared Living Program Manager.
ESSENTIAL FUNCTIONS:
Assist the Shared Living Program Manager with a variety of responsibilities.
Adhere to all expectations and protocols for the Shared Living program.
Possess a functional knowledge of individual person-centered plans.
Support the Shared Living team to ensure completion and compliance with all OADS requirements.
Communicate effectively and professionally with co-workers and other Creative Works personnel.
RESPONSIBILITES:
Support the Shared Living Program manager by completing historical summaries for new individuals.
Assist with completing Personal information and billing payroll forms.
File and track all forms and documents for the individuals in Shared Living as requested.
Complete and update the releases for Shared Living programs.
Attend Person-centered planning meetings and enter service plans into the Evergreen system.
Enter and update service plans as needed or requested.
Complete narratives for new Shared Living programs.
Send and ensure completion of rental agreements for all Shared Living programs.
Assist with submitting Assistive Technology requests.
File documents into Shared Living provider binders and ensure they are current.
Provide maintenance to ensure required documents remain up to date.
Carry a caseload as needed or requested with a maximum of 8 at any given time.
QUALIFICATIONS:
High School Diploma or GED.
Must have experience or be willing to learn the OADS Evergreen system.
Valid U.S. driver’s license with acceptable driving record.
Experience and/or knowledge in supporting individuals with disabilities.
Must have or be willing to complete the CDS curriculum if you do not have it.
Experience or knowledge of the Shared Living model preferred.
Must have the ability to travel for home visits and/or meetings.
Excellent communication and interpersonal skills.
Must possess adequate writing skills.
PHYSICAL WORK TOLERENCES:
While performing this job the employee will be required to sit for extended periods of time.
