Program Overview – Homes for Good (St. Hilda’s) – Supportive Housing, Community Care Unit
The Supportive Housing Program at WoodGreen provides stable housing and comprehensive supports for seniors who have experienced street involvement and may face mental health or addictions challenges. The program offers a wide range of onsite services, including psychosocial and primary care supports, intensive case management, harm reduction and addiction services, therapeutic and supportive counselling, 24‑hour staffing, assistance with daily living, nursing services, life‑skills development, recreational activities, volunteer opportunities, and follow‑up support. This work is grounded in WoodGreen’s mission to promote wellness, self‑sufficiency, poverty reduction, and strong, sustainable communities.

Working as part of a multidisciplinary Supportive Housing team, this position plays an essential role in supporting individual clients and fostering community engagement. The role involves collaborating closely with internal partners while occasionally networking with external organizations to expand programming and enhance opportunities for tenants. Through this teamwork‑driven environment, the Coordinator contributes to creating an inclusive, supportive, and enriching living experience for residents.

The Recreational Program Coordinator is responsible for developing, organizing, facilitating, and evaluating social, recreational, and educational programs that promote wellness among senior tenants. The role includes assessing emerging client needs to develop new initiatives, supervising students and volunteers to strengthen program delivery, and providing administrative support to ensure high‑quality service. Additionally, the Coordinator assists the Manager in developing policies and procedures that enhance program effectiveness and support the continuous improvement of service delivery systems.

What You Will Do
  • Plan, build, organize & facilitate onsite activities onsite staff and external partners
  • Engage with tenants to complete an interest assessment of what types of programming tenants would be interested in partaking in
  • Assess, develop, and build healthy and engaging program spaces
  • Facilitate skill-building, healthy living and fun workshops for participants.
  • Create a calendar of planned monthly programming while liaising with external parties and supporting staff with the development and co/facilitation of daily/weekly workshops & activities
  • Create a participant intake pathway & marketing materials to inform tenants and get them excited
  • Create & implement monthly/quarterly Pre & Post-program surveys
  • Interact with individuals in person, providing emotional support, and feedback to case managers/counsellors and encouraging participation in programs.
  • Work with the case manager to carry out goals and expectations of the care plan and complete daily needs assessments to determine appropriate programming for participants
  • Monitor clients, identify additional needs and mitigate risks
  • Identify participants who may be interested in becoming peer helpers
  • Collaborate with the community and build partnerships to bring in new programming
  • Work collaboratively with community partners including mental health, substance use, geriatric crisis and mental health and justice, to maximize support of client and help create pathways to recovery goals
  • Ensure that anti-oppression, community development, and harm reduction principles are achieved in the program
  • Carries out additional duties as identified from time to time by Management.
  • Take part in team and unit meetings
  • Participate in ongoing training and education
  • Participate in regular supervisory sessions and annual performance appraisals
  • Help to create a healthy and motivating work environment
  • Recognize that the programs, staffing model, and job description may be reviewed after the first year of operation and the position/ responsibilities may need to change following that review
  • Document client interactions on provided software
  • Collect, input and extract statistical information using provided software
  • Be prepared to work in environments where they might be pest control and hoarding issues
  • Be prepared to work in environments where there may be death and dying
  • Be prepared to work in environments where there may be drug use

What You Bring to the Team
  • Bachelor-level degree or Social Service Worker with 5+ years’ experience creating and facilitating programming
  • 5+ years of related experience working with adults/seniors, homeless individuals with mental health/substance use issues
  • Demonstrated experience supporting adults/seniors with mental health, substance use, housing needs, etc.

  • Comprehensive understanding and knowledge of the health sector and its related services
  • Excellent knowledge of community resources
  • Demonstrated ability to follow policies, procedures, and adhere to legislation.
  • Well grounded; good boundary setting skills
  • Travel may be required while on shift – community building/partnership meetings
  • A thorough understanding of clinical practices and care models.
  • Ability to exercise good judgment, flexibility, and contribute to the effective functioning of a clients
  • Strong leadership skills
  • Proven ability to facilitate presentations and workshops.
  • Applied computer skills (MS word, excel, and Internet).
  • Ability to use case management software is an asset (Pirouette).
  • Ability to work weekends & statutory holidays on an exceptional basis, where and if applicable
  • Might work in environments where clients use substances.
  • Might work in environments wherein clients have incontinence issues.
  • Clients may exhibit responsive behaviours including profanities and expletives.
  • Able to lift 40lbs.
