Program Overview:
Enhanced Care Management (ECM) is designed to provide comprehensive, coordinated care to Medi-Cal beneficiaries with complex health and social needs, such as those with multiple chronic conditions, experiencing homelessness, or facing significant behavioral health challenges. It aims to improve health outcomes through a person-centered approach that includes intensive care management, addressing physical, behavioral, and social determinants of health.
Job Description:
The ECM LCSW will provide clinical oversight and/or clinical supervision to the ECM non-clinical Lead Care Managers (Case Managers) and clinical team including associate social workers and/or associate therapists, ensuring the delivery of quality care and compliance with program, contract, and ethical standards. This role involves supporting the mission of the organization, acting as a clinical resource, and overseeing the implementation and monitoring of care plans to achieve desired patient outcomes. The LCSW collaborates with the patient’s interdisciplinary team (ICT) to support the patient and engage their support systems to define priorities central to the member’s needs and goals.
Requirements:Clinical Oversight:
- Provide clinical oversight to non-clinical Lead Care Managers (Case Managers).
- Act as a clinical resource for the ECM care team, as needed.
- Supervise the provision of services to ensure high standards of care.
- Support differential thinking when symptoms are unclear
- Actively participates in ICT meetings, offering clinical consultation, and risk assessment guidance to support coordinated, whole-person care.
Care Plan Management:
- Review to Care Plans ensure appropriate clinical goals and interventions.
- Oversee and monitor care plans to ensure correct implementation and achievement of desired outcomes.
- Recommend revisions or updates as necessary to accomplish the patient’s goals.
Patient and Family Support:
- Provide psychoeducational information as appropriate for patients and/or family members or accompanying caregivers.
- Provide therapeutic interventions.
- Provide emotional support and counseling to patients and their families during crisis situations.
- Facilitate communication and problem-solving to address the immediate needs and concerns of patients and families during crisis situations.
Training and Development:
- Provide training to staff as needed.
- Participate in all meetings and trainings as assigned.
Collaboration:
- Work effectively and cooperatively with all staff and external partners.
- Participate in interdisciplinary team meetings to ensure coordinated care.
- Work in close harmony with the ECM administrators, accepting direction and implementing policies and procedures reflective of this direction.
Quality Assurance:
- Administer quality assurance procedures, including record reviews.
- Monitor program performance and outcomes to make recommendations for improvements.
- Thoroughly, accurately, and promptly document all services rendered in the electronic medical record for every patient.
- Participate in quality improvement projects as assigned.
Cultural Sensitivity:
- Display sensitivity to the service population’s cultural and socioeconomic characteristics.
Safety and Compliance:
- Adhere to the National Association of Social Workers (NASW) Code of Ethics.
- Follow all policies, procedures, and legal mandates, including HIPAA, member confidentiality, releases of information, protected health information (PHI), and member files.
Education and Qualifications:
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current and unrestricted license as a Clinical Social Worker (LCSW) in the state of California
Experience and Skills:
- Minimum of 2 years of experience in a clinical or healthcare setting.
- Experience working with diverse populations and complex cases.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary team.
