Responsible for providing comprehensive pharmacy services to support safe, effective and evidence-based medication use across inpatient, outpatient, infusion center and emergency settings in a small rural hospital located in “Alaska’s Playground.” This position works collaboratively with physicians, nurses, and other healthcare professionals to optimize patient outcomes and ensure compliance with regulatory standards and best practices.
Essential Duties and Responsibilities:
• Review, verify and dispense medications to ensure accuracy, appropriateness and safety
• Provide clinical consultation and drug information to healthcare providers and patients
• Supervise pharmacy technician activities
• Monitor medication therapy, labs, dosing and pharmacokinetics to optimize outcomes
• Participate in patient care rounds
• Participate in antimicrobial stewardship, diversion, quality and medication safety initiatives
• Prepare and dispense sterile, non-sterile and hazardous medications (including chemotherapy/hazardous medications)
• Oversee medication reconciliation, accurate documentation and maintenance of current medication lists
• Respond to code blue and emergency situations
• Participate in quality improvement, staff education and continuing professional development
• Ensure adherence to hospital, state and federal pharmacy standards
• Perform other duties as assigned
REQUIREMENTS:
Bachelor’s or Doctorate in Pharmacy
AK Pharmacist Licensure or eligibility to obtain
BLSP within 30 days
Strong clinical knowledge, communication skills and ability to work independently
Collaborative team player with strong relationship building skills
PREFERRED:
Two year of Pharmacy-Hospital experience
One year of Oncology-Infusion experience
Advance Cardiac Life Support (ACLS) certification (or eligible to obtain)
Completion of PGY 1 and/or PGY2 pharmacy residency
