Job Summary

• The Community Health Workers (CHWs) are public health workers who have a close understanding of the community they serve.
• This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
• This individual would work closely with medical providers, primary care teams, and social service agencies to provide care coordination and connections to improve their health and general well-being through education and provision.
• This individual would work in both clinical and community-based settings.

Education and/or Experience

• High School Diploma or Equivalent required
• A minimum of two years’ direct experience preferred.
• Experience with navigating local medical and social support systems preferred.
• Possible bilingual and medical interpreter qualifications preferred based on specific position needs.
• Possible knowledge and/or experience with certain medical diagnoses preferred based on specific position needs.
• Must be from the community that is being served or have a familiarity of the community.

License/Certification/Registration

• This position requires a valid driver’s license and that, the employee is insurable by the Avera Health automobile liability insurance carrier.
• Must complete the South Dakota Community Health Care Worker (CHW) Certification within 2 years of hire and maintain certification.

Work Schedule

• Day Shift: Monday-Friday; 80 hours per 2 weeks
• Occasional weekends and evenings for special events or speaking engagements.

Pay

The hourly range for this position is listed below. The actual hourly rate is dependent upon experience.

• $19.25 - $24.25 per hour
