We are looking for a Spiritual Care Coordinator to service our New River Valley and Roanoke area on our Home Hospice Team!


Duties & Responsibilities:

· Conducts initial spiritual assessments to identify spiritual needs of our patients and family members and incorporate this into the individualized plan of care.

· Provides effective interventions on initial and subsequent visits to meet the unique goals of our patients and family members.

· Provides outreach and education/support to facilities and other community resources

· Maintains relationships with local spiritual /faith providers and resources

· Serves as spiritual support for staff regarding issues of grief and loss as it related to their professional roles

· Participates in Ethics and other relevant committees to enhance our overall program

· participates in the ongoing development of the role of Chaplain within the agency and within community facilities


Job Requirements:

· Master’s Degree in Divinity or related field required. Degree must be from an accredited program.

· 4 units of Clinical Pastoral Education (CPE) preferred

· Board Certification preferred

· Two years of experience in spiritual care/pastoral counseling

· One year of chaplaincy work with dying or grieving individuals preferred

· Strong ability to respect each patient and family’s unique spiritual and cultural beliefs

· Ability to work effectively in private homes, facilities and hospitals

· Ability to organize, prioritize, manage time well and juggle multiple responsibilities simultaneously

· Skilled in crisis intervention

· Strong communication and presentation skills

· Excellent computer skills

· Virginia driver license, good driving record and reliable transportation
