Job Description
A Clinic Registered Nurse (RN) or Licensed Practical Nurse (LPN) works under the general supervision of the Physician and/or Nurse Practitioner, performs various patient care activities and related non-professional services necessary in caring for the personal needs and comfort of the patients. Assists in the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation with consideration of the aging process, human development stages and cultural diversity. Assists in the care and preparation of clinic/surgical patients in various clinic settings. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.
This position is also responsible for chronic care management (CCM) of established clinic patients.
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Maintain confidentiality of all patients, hospital, or physician related information.
- Focuses on quality standards of care.
- Follows guidelines of safety (incident reporting) and staff health/infection control (illness reporting)
- Is knowledgeable of general clinic policies and procedures and assists with their review and updates.
- Ensures that the organization’s Compliance Plan and all regulatory rules and requirements are followed.
- Makes clinical decisions and sets priorities related to workload management based on clinical knowledge and individual patient needs.
- Proactively participates in processes to guide patient care in the most efficient, productive, cost-effective manner possible.
- Work as part of the healthcare team to assess patient needs, plan and modify care and implement interventions.
- Establish and maintain effective working relationships with patients, providers, team members, insurance companies and the public. Work with peers and providers in a team environment.
- Obtain lab, x-ray and other reports and results, hospital notes, referral information, consults, etc. Monitor, track and resolve outstanding orders, referrals, consults, labs, procedures, etc. in EMR.
OTHER RESPONSIBILITIES
- Displays accountability for actions and decisions for the continuity of patient care.
- Collect and document in-depth information on the patient’s situation. Assess patient’s health, medical, legal, and psychological needs for services through contact with the patient, community or professional caregivers, family and other support systems and medical record review, to develop a case management plan. Reassess ongoing needs for services. Communicate/interact with patients and families/support systems, as well as others in the healthcare system such as physicians, third party payors, community resources/agencies, the multidisciplinary team, and other hospital departments to meet the holistic needs of the patient.
- Determine objectives, goals, and actions to meet patient identified needs, including desired outcomes with set timeframes utilizing interventions appropriated for pediatric, adolescent, adult and geriatric patients.
- Organize, secure, integrate and modify resources necessary to accomplish goal in the plan. Communicate needed resources to physicians and service organizations.
- Collect ongoing information from all sources concerning the plan’s interventions to determine the plan’s effectiveness. Coordinate multi-disciplinary team meetings.
- Periodically determine the plans effectiveness in meeting the desired outcome and goals and modify the plan as indicated.
- Coordinate care, implement strategies, and conduct prospective, concurrent, and retrospective reviews based on established evidence-based guidelines.
- Participate in improvement teams to change processes and systems to achieve seamless care delivery.
- Conduct follow-up evaluation of care across continuum of care and patient satisfaction identifying clinical, operational, financial, or social issues impacting patient outcomes.
- Makes clinical decisions and sets priorities related to workload management based on clinical knowledge and individual patient needs.
- Completes assigned daily duties.
- Assists with daily, weekly, and monthly cleaning of equipment, work, and storage areas.
- Accepts reassignment to other units if necessary.
- Follows expected work practices.
- Displays thoroughness and accuracy of work.
- Works in a safe manner, including reporting unsafe equipment or environment.
- Well organized, accepts assignments willingly and accomplishes them efficiently.
- Anticipates problems and suggests solutions.
- Help as needed with duties not specifically assigned.
- Works steadily and always keeps busy.
- Maintain knowledge and skills necessary to communicate, teach and interact with patients, visitors, and staff in the following age groups:
- Infant
- Pediatric/adolescent
- Adult
- Geriatric

Requirements:EDUCATION AND/OR EXPERIENCE
- Associates Degree required.
- Ability to pass specific competencies for this position.
- Proficient oral and written communication.
- 1 year office experience preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
- Healthcare professional currently licensed in the state of Illinois (Registered Nurse or Licensed Practical Nurse)
- CPR required or be willing to obtain.
- Additional certifications may be required.
PHYSICAL DEMANDS
- Prolonged, extensive, or considerable sitting, standing, and walking.
- Required to climb, balance, stoop, and kneel.
- Required to use arms, hands, and fingers for repetitive grasping, pulling, and pushing and feeling.
- Frequently lift and/or move up to 25 lbs. and occasionally lift and/or move up to 50 pounds. Any workload exceeding 50 pounds will require assistance.
- Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus.
WORK ENVIRONMENT
- Works in a well-lit, climate controlled, office setting.
- Eyestrain and other conditions from use of computer screen may occur.
- May be exposed to communicable diseases, hypodermic/IV needles, unpleasant sights, odors, and materials.
- The noise level in the work environment is quiet to moderate.

Understands the responsibilities related to compliance and knows how to contact the Corporate Compliance Officer should there be any instance of question or concern regarding fraud and/or abuse.

SALARY RANGE
RN $41.25 - $46.25 PER HOUR
LPN $29.00 - $34.00 PER HOUR
