Client Case Manager – Position Summary
The Client Case Manager is responsible for managing a caseload of individuals participating in Ascend Services programs. This role focuses on developing and delivering support services that empower clients, connect them to meaningful employment and community opportunities, and inspire personal growth. Acting as a key liaison, the Client Case Manager collaborates with clients, guardians, mentors, service providers, and other stakeholders to ensure individualized support and success.
This position is ideal for an individual who is passionate about client advocacy, community integration, and personal empowerment, with the ability to provide both structured case management and hands-on support in a dynamic environment.
Key Responsibilities:
- Develop person-centered plans tailored to each client’s goals and needs.
- Maintain accurate case records and complete all required documentation, including monthly reports and client satisfaction surveys.
- Serve as the primary point of contact for clients, families, mentors, and external service providers.
- Support clients in developing soft skills and meeting work performance expectations.
- Promote the development of natural support systems to foster peer relationships and community integration.
- Ensure compliance with funding source contract specifications and program guidelines.
- Provide direct support and personal care as outlined in the client’s service plan.
- Respond promptly to emergency situations, administering First Aid or CPR/AED as needed.
- Uphold strict confidentiality regarding client, staff, and business matters.
- Engage in ongoing training and professional development to stay informed on best practices.
- Collaborate with the Program Support Coordinator and Program Director to report incidents and ensure quality service delivery.
Qualifications & Skills:
- Bachelor’s degree in Human Services or a related field.
- Experience in the rehabilitation field is preferred.
- Strong leadership, organizational, and problem-solving skills.
- Excellent verbal and written communication abilities.
- A commitment to serving others and fostering a supportive, inclusive environment.
- Proficiency in computer operations and case management systems.
Work Environment & Physical Requirements:
- Split work schedule: 40% office-based and 60% direct client service.
- Ability to stand for extended periods and physically assist clients, including lifting up to 50 lbs. with assistance.
- Must be able to bend, squat, twist, and kneel frequently.
- Must pass a pre-employment drug screen, physical, and TB test.
- A valid driver’s license is preferred; must meet Ascend Services’ vehicle insurability standards if transporting clients.
