HOPE SEES/Case Manager at Children Service Center – Honesdale, Pennsylvania

The Supported Employment/Educational Specialist (SEES)/ Case Manager will work with transition age youth and young adults, who are between the ages of 15 and 30 to develop strategies needed to enhance Independent Living Competencies such as Social Skills, Personal Adjustment, Vocational Skills and Educational Skills. You will be provided with a flexible schedule that depends on the need of the client. This team member will be fully trained in daily activities and the use of community resources. This position will report to HOPE program director or designee. Closely monitoring and advocating for services as needed by transition age youth and young adults 15-30 years old. Provide on call coverage for the BCM department through their rotation.

Responsibilities Include, but are not limited to:

• Knowledge of the mental health and social service field
• Ability to use good judgment skills in assessing consumer needs
• Intensive case manager must become familiar with available community resources and keep up to date on their locations, eligibility requirements and program changes

Qualifications:

• Bachelors Degree in Social Work, Sociology, Psychology, related social science field
• Two years experience in Mental Health Services, Vocational field, or Educational Support preferred, or equivalent combinations of experience and training in a public or private human services organization
  • Experience in working with children/adolescents and their families preferred
  • experience in Mental Health Services, Vocational field, or Educational Support preferred
  • Must obtain Case management, Individual Placement Services (IPS), and Student Assistance Program (SAP) certifications upon hire.
