Company Summary
Bridge House is the premier diagnostic and assessment-oriented residential treatment center for adults. Set in a comfortable home-like environment, Bridge House provides stabilization, assessment, diagnostics, and treatment for adults struggling with mental illness. Each component of the program is designed to assist individuals in gaining a better understanding of what it means to be them and is implemented to aid in acquiring the skills required to live a life with purpose and fulfillment. Bridge House is currently expanding to provide continued care services and is seeking qualified part-time Executive Functioning Coaches to add to the team.

Schedule includes 1-2 weekday shifts (Mon-Fri) and 1 weekend shift (Sat/Sun).

Primary Responsibilities
The Executive Functioning Coach/Case Manager, in collaboration with the Program Director, physicians, and other health practitioners, will be responsible for implementing life skill development plans for individuals with mental or emotional conditions or disabilities. They will also monitor residents’ physical and emotional well-being and report to medical staff and the treatment team. They will participate in rehabilitation and treatment programs and help with activities of daily living.

Job Duties
• Maintain a safe therapeutic milieu by conducting regular checks on residents, resident rooms, resident inventory, and facility property so residents can focus on managing their illnesses and disabilities to promote healing.
• Monitor residents' physical and emotional well-being and report unusual behavior or physical ailments to medical staff and the treatment team.
• Identify and create an individualized care plan for residents, in collaboration with the Program Director, which includes executive functioning skills, independent living skills, activities of daily living, etc.
• Assist in the implementation of the individualized care plan.
• Encourage and aid residents to develop their own activities of daily living, practice social skills and to participate in social, recreational, or other therapeutic activities that enhance interpersonal skills or develop social relationships.
• Complete necessary resident charting and documentation within each shift.
• Observe and influence residents’ behavior, communicating and interacting with them and teaching, counseling, or connecting with them.
• Collaborate with or assist mental health therapists, psychologists, or rehabilitation therapists in working with Bridge House residents.
• Train or instruct new employees on procedures to follow with psychiatric patients.
• Develop or teach strategies to promote resident wellness and independence.
• Lead prescribed individual or group therapy sessions as part of specific therapeutic procedures.
• Maintain personal and professional boundaries while upholding program policies and procedures.
• Transport and closely supervise residents at appointments or off campus activities and report behavioral observations upon returning to the facility.
• Attend monthly staff meetings and mandatory trainings.
• Read all required correspondence related to resident care prior to each shift.
• Demonstrate understanding of Bridge House policies and procedures.
• Administer medication under the direction of medical and nursing staff.
• All other duties as assigned.

Qualifications/Skills
• Bachelor's degree in the human services field preferred.
• Must be 21 or over.
• Must have a valid drivers license and 5 years driving experience with a current good driving record.
• Must pass background check.
• Must be punctual and reliable in attendance and follow shift coverage protocols. 95% attendance to scheduled shifts is expected.
• Must be able to exert 50 pounds by lifting, pushing, carrying, etc.
• Physical requirements include bending, carrying, far
ear vision, lifting, hearing/listening, manual dexterity, pulling, pushing, reaching, sitting, speaking, standing and walking for distances and on uneven terrain, including up and down stairs.
• Must possess physical stamina to stand for long periods of time while providing supervision, moving with residents, and be able to perform extended, sustained physical safety restraints when needed.
• Must be able to listen, follow directions, and communicate resident location, status, and other information using a Bridge House radio and earpiece.
• Must be able to stay attentive and alert for extended periods of time.
• Must have the ability to build rapport and connect with diverse people from different backgrounds.
• Must be able to demonstrate warmth, empathy, and patience to our residents.
• Excellent verbal communication skills are required.
• Problem solving, quick thinking, good memory, and active listening skills are essential.
• Must be able to work as a team and possess good stress management skills in order to remain calm amid chaos.
• Basic computer skills are required for charting, scheduling, and email functions.
