Job Summary

The Licensed Vocational Nurse (LVN) is responsible for providing direct patient-centered nursing care under the supervision of a Registered Nurse or physician. The LVN contributes to the coordination of care to achieve desired patient outcomes, participates in patient and family education, and supports the healthcare team in maintaining standards of professional nursing practice. The LVN demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patients served.

Essential Functions

  • Obtains pertinent patient information, including medical history and current condition, in a professional and objective manner.
  • Communicates patient care information effectively throughout the shift and during handoff to appropriate personnel.
  • Documents all aspects of patient care accurately and timely in the electronic medical record.
  • Assists in the development and coordination of patient care plans in collaboration with the Registered Nurse.
  • Administers medications safely and accurately in accordance with physician orders and facility policies.
  • Monitors patients and reports changes in condition to the appropriate healthcare team members.
  • Identifies and addresses skin integrity concerns, particularly in geriatric and immobile patients, and implements appropriate interventions.
  • Ensures proper use of restraints, including verifying physician orders and completing required documentation.
  • Uses clear, concise, and professional communication with patients, families, and healthcare team members.
  • Provides basic patient education and reinforces instructions as directed by the Registered Nurse.
  • Maintains compliance with HIPAA regulations and patient privacy standards.
  • Utilizes appropriate communication tools such as SBAR during shift handoff and when reporting patient information.
  • Maintains a clean, safe, and organized patient care environment.
  • Assists with patient care activities and supports the interdisciplinary team as needed.
  • Attends required trainings and staff meetings.
  • May float to other departments as assigned.
  • Performs other duties as assigned.

  • Attends staff meetings; follows HIPAA guidelines; follows patient rights policy; complies with the compliance program; demonstrates knowledge of role in a disaster; demonstrates knowledge of fire and fire drill procedures; maintains working knowledge of hospital emergency codes; utilizes standard precautions; and safely manages the environment of care in accordance with all safety and regulatory programs.
Requirements:

Experience

  • Recent acute care hospital experience
  • At minimum 1 month of LVN experience
  • Communication, organizational, computer skills, team player, work independently and quickly
  • Must be able to properly read EKG.

Education

  • Graduate of an accredited school of vocational nursing.
  • Current Licensed Vocational Nurse (LVN) license in the State of Texas.
  • Current BLS certification required (AHA).
