Position: Lead Care Manager

Compensation: $57,000 to $60,000 per year; non-exempt position

Location: Riverhead, NY and travel throughout Suffolk County

Hours: 37.5 hours per week

Start Date: Immediately

Position Description:

This position within our Adult Care Management Program will cover a variety of duties and responsibilities that are comprised of a combination of supervisory, mentoring and administrative tasks and require the demonstration of leadership carried out in a manner consistent with our agency’s overall mission.

In addition to those job functions and responsibilities listed below, additional job-specific responsibilities shall be articulated:

  1. By the Adult Care Management Program Director as discussed and refined in mutual consultation;
  2. By the Health Home(s) Policy and Procedures manual;
  3. MHAW Personnel Policies and Corporate Compliance Policies;
  4. Employee handbook;
  5. Diversity, Equity and Inclusion Statement.

The Adult Care Management Team Coordinator shall act as the immediate supervisor of the Lead Care Manager. The Adult Care Management Program Director will act as supervisory back-up for the Team Coordinator.

Key responsibilities will include, but are not limited to:
  • Maintain a blended caseload; includes Health Home Plus members with Serious Mental Illness (SMI).
  • Proficient in conducting appropriate screenings/assessments (i.e. Comprehensive Assessments, Risk Assessments).
  • Coordinate Care Management needs and successfully engage high need individuals by utilizing a recovery-oriented approach and skills such as motivational interviewing and trauma-informed care.
  • Mentor new ACM staff: Provide oversight and monitor productivity of New ACM Care Managers; Provide support to new team members as they navigate IT platforms (i.e. Foothold), workflows, policies and procedures.
  • Identify performance issues/training needs and bring to Team Coordinator’s attention.
  • Assist with the monitoring of team’s services each month; assist with providing services as needed.
  • Assist Team Coordinator with the oversight of special projects.
  • Participate in staff meetings; help to facilitate team meetings and other administrative meetings/trainings as required.
  • Perform duties with designated amount of time in community and office setting.
  • Respond to and supports Care Managers with urgent client situations in the office and community, as needed. Utilize crisis intervention skills and access community supports (i.e. Mobile Crisis, DASH) when necessary.


Qualifications:
  • Master’s Degree in a human service/health discipline, license preferred.
  • 4+ years of Care Management and leadership experience preferred
  • SIFI Certification a plus
  • Bi-lingual, English/Spanish is a plus; stipend is awarded for assessed and verified bi-lingual proficiency
  • Strong collaborative documentation skills
  • Working with those who have significant mental health and chronic physical health conditions.
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help.
  • Self-starter with the ability to work both independently and as part of an interdisciplinary team; with measurable results.
  • Critical thinker with exceptional leadership, communication, time management and organizational skills.
