Job Title: Care Coordinator

Reports To: Assistant Executive Director

FLSA Status: Non-Exempt

Position Summary

This position supports the Executive Director by assisting with resident care coordination, medication processes, documentation, supply management, and wellness operations while also functioning in conjunction with the Resident Assistant/CNA role.

Essential Duties and Responsibilities

Core duties and responsibilities include, but are not limited to, the following:

Resident Wellness & Alarm Support

  • Assist with resident alarm systems including call pendants, bed/chair/motion/door alarms.
  • Install, activate, deactivate, inspect, troubleshoot, and replace alarms as needed.
  • Ensure timely setup and removal based on resident needs and move-outs.
  • Document all completed wellness tasks accurately.

Medication & Clinical Support

Under the guidance of the Executive Director:

  • Destroy expired or discontinued medications following facility procedures and dual-staff verification.
  • Inventory and organize diabetic supplies and medical products upon delivery.
  • Order PRN medications, narcotics, and bulk items as directed.
  • Maintain clean, organized medication carts, wellness rooms, and supply areas with weekly checks.
  • Maintain sharps containers and coordinate replacements.

Documentation & Quality Monitoring

  • Review ECP daily for care and documentation gaps and report issues for correction.
  • Complete monthly cycle fills and communicate discrepancies with pharmacy and leadership.
  • Prepare quarterly psychotropic medication review documentation for physician review.

Communication & Quality Initiatives

  • Participate in caregiver meetings to promote collaboration across departments.
  • Participate in Falls Committee meetings to identify root causes and implement prevention strategies.
  • Communicate medical supply needs to leadership and ensure proper storage.

Compliance & Professional Standards

  • Follow all company policies and procedures.
  • Maintain compliance with DHS 83 and DHS 89 regulations as applicable.
  • Attend required meetings, trainings, and educational programs.
  • Maintain a professional appearance and demeanor at all times.

General Responsibilities

  • Perform additional duties as assigned by the Executive Director, Lead Care Coordinator, or Director of Community Relations.

Requirements:

Qualifications

Education and Experience

  • High school diploma or equivalent required.
  • Associate’s or Bachelor’s degree in healthcare, business, or related field preferred.
  • Experience in senior living, healthcare, or wellness operations preferred.

Certificates, Licenses, and Training

  • CBRF certified training in First Aid/Choking, Fire Safety, Standard Precautions training preferred.
  • Must hold CBRF Medication Administration training and have experience in administering medications in a CBRF setting.
  • Experience working with older adults preferred.

Schedule and Availability

  • Flexible schedule based on community needs.
  • On-call for scheduling every 7 weekends approximately.
  • Occasional travel between communities may be required.
