Clinical Navigator, Stroke at White Plains Hospital – White Plains, New York

Position Summary

The Clinical Navigator (Neurosciences) will serve as a single point of contact for patients, caregivers, and providers to support the entire stroke and neurovascular care continuum from pre-hospital, admission/post-procedural recovery, to discharge planning and follow-up outreaches. The Clinical Navigator may also be involved in the outpatient practice and community outreach events. The Clinical Navigator improves patient preparedness for treatment and recovery through education and psychosocial support while also facilitating interaction between patients/caregivers and their provider(s). The Clinical Navigator assists in the development of appropriate educational materials and program monitoring to promote coordination of care and patient experience. The position also assists the leadership team with program development, expansion, and successful achievement of programmatic and strategic goals. The Clinical Navigator adheres to any standards as outlined by The Joint Commission, the American Heart Association, the American Stroke Association, etc.

Essential Functions and Responsibilities Includes the Following:

1. Assists with all clinical and administrative aspects, initiatives, and responsibilities associated with the Stroke program.
2. Follows patients throughout the continuum of care, working to ensure seamless care and coordination of services.
3. Facilitates timely transition between pre-hospital, admission, post-procedure recovery, discharge planning, and outpatient follow-up appointments.
4. Works as a link between the patients and the other members of the support care services (e.g. social work, nutrition, transportation, financial counseling, research, etc.)
5. Develops patient education programs and tools.
6. Connects the patient and their family to the available community resources as needed.
7. Maintains required metrics, data and statistics and report them monthly and/or as required and participates in quality improvement activities relevant to program and role.
8. Attends weekly Neurovascular Case Conference, monthly Stroke Meeting, and any other relevant meetings as required.
9. Understands and adheres to the WPH Performance Standards, Policies and Behaviors.

Performs all other related duties as assigned.

Education & Experience Requirements

• Registered Nurse in New York State
• BSN required; MSN preferred
• Five years of stroke/neurosciences experience strongly preferred
• SCRN or CNRN certification preferred
• ENLS certification preferred
• Spanish speaking preferred

• May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospital’s SDS (Safety Data Sheet) data base and may be accessed through the hospital’s Intranet site (Employee Tools/SDS Access). A copy of the SDS data base can also be found at the hospital switchboard, saved on a disc.
