Acute Care Liaison at Suncoast Center – St. Petersburg, Florida

Minimum Qualifications:
- A bachelor's degree from an accredited college or university in any field.
- 1 year work experience providing at least 12 months of related case management or Crisis stabilization services to either the adult or child population.
Acute Care Liaison responsibilities include but are not limited to:
- Identifies potential Suncoast Center clients at Eleos and/or Crisis Stabilization Units in Pinellas County.
- Facilitates follow up appointments of current and potential clients within seven days of discharge.
- Assists with linking clients to securing of benefits, housing, medications, other financial services, and any other needs identified at the time of admission.
- Timely assessment of clients upon admission to the Crisis Stabilization Unit (CSU) to allow for the maximum time available to procure whatever goods or services are needed.
- Works on initial engagement with clients who may not believe that it is in their best interest to receive follow up services.
- Will provide information about the agency and information about client’s mental health in an effort to encourage participation in their recovery.
- Will meet the individuals who have been admitted to Crisis Stabilization units and determine if there are services that could be provided to them to divert them from the CSU.
- The ultimate goal of this position will be to improve follow up with community agencies thereby reducing recidivism to the CSU.
- Facilitate the admission and discharge groups as needed in the CSU.
- Will utilize electronic medical records system with Suncoast Center and Eleos to facilitate coordination of care.
- Completion of all documentation of these services as required.
- Utilize data reports to monitor the show rate of client referrals.
- Facilitate enrollment and intakes to Suncoast Center Programs.

Working Conditions: Work is in office. Schedule may be irregular if emergencies arise.
Job Requirements:
- Must be 21 years of age.
- Must have a negative drug screening prior to hire and throughout employment.
- Must pass a Criminal Background Check, local checks, and DCF Level II Background Screening prior to hire and annually.
- Must have valid Florida Driver’s License and pass motor vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character).
