Full-time Hospice LPN Position

Schedule: Monday thru Friday, 8a-5p

Available territory openings: Mishawaka and surrounding communities

Performs physical and psychosocial assessment on all hospice patients.

Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.

Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.

Assesses patients condition and coordinates with RN Case Manager.

Participates in case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.

Experience:

One year Hospice experience preferred, but not necessary

1 year of LPN experience required

Credentials:

Licensed LPN in State of Indiana

Current driver's license

Must have reliable transportation and insurance
