Thrive Navigator

  • Position Summary
    • The Thrive Iowa Navigator provides family-centered navigation services to individuals and families facing social, economic, or health-related barriers. Acting as a trusted guide and advocate, the Navigator helps families move from crisis to stability by coordinating care, identifying needs, and connecting them to community resources. This role strengthens community-based systems of care by improving access to services, supporting whole-family well-being, and fostering collaboration across partner agencies in close coordination with the Thrive Iowa Coordinator and care teams.

  • Education and Experience Requirements
    • EDUCATION:
      • Required:
          • Bachelor’s degree in social work, public health, human services, psychology, education, or a related field, or equivalent relevant experience.
      • Preferred:
    • LICENSES AND CERTIFICATIONS:
      • Required:
          • Valid driver’s license
            • This position requires some travel
      • Preferred:
    • TRAINING AND EXPERIENCE:
      • Required:
          • Experience working with families, children, or vulnerable populations preferred.
          • Strong communication, organizational, and documentation skills.
          • Ability to work independently while collaborating within a multidisciplinary team.
          • Proficiency with electronic case management systems or willingness to learn.
      • Preferred:
          • Experience in care coordination, case management, navigation, or community-based services.
          • Training or experience in Motivational Interviewing or trauma-informed care.
          • Familiarity with local community resources and social service systems.
          • Bilingual or multicultural experience is a plus.
  • Essential Job Functions
    • Participant Intake & Engagement

      • Conduct initial intake appointments, including comprehensive assessments, consent collection, and enrollment documentation.
      • Maintain an active caseload of approximately 50–75 participants, serving an average of 100+ families annually.
      • Meet with families in a private office setting; alternative meeting locations may be used when requested, while prioritizing confidentiality.
      • Establish trusting relationships with families through trauma-informed, strengths-based, and culturally responsive practices.

      Ongoing Support & Case Management

      • Develop individualized action plans based on participant needs, goals, and priorities.
      • Update participant assessments at least every 30 days.
      • Make referrals to appropriate community resources and assist families in navigating systems and removing barriers to access.
      • Maintain regular communication with participants and providers, ensuring timely follow-up and continuity of care.

      Documentation & Data Management

      • Document all contact attempts, identified needs, referrals, and progress notes in HopeHub and related case management systems.
      • Routinely review case data for accuracy, completeness, and consistency.
      • Ensure compliance with all data privacy and confidentiality requirements, including HIPAA, FERPA, and COPPA.

      Care Team Collaboration

      • Identify and engage appropriate care team members to support participant needs.
      • Participate in multidisciplinary meetings, including alliance meetings, one-on-one supervision with the coordinator, and monthly Primary Provider meetings.
      • Attend additional meetings (e.g., court hearings, Team Decision Making meetings, CPS staffing) as needed to support families.

      Training & Professional Practice

      • Apply Motivational Interviewing techniques and other evidence-informed strategies in daily practice.
      • Complete all required training, including HopeHub LMS, Case Management, Mandatory Reporter, HIPAA, and Child Welfare training.
      • Participate in shadowing and peer learning opportunities as part of ongoing professional development.
      • Uphold ethical standards and professional conduct in accordance with Primary Provider and Thrive Iowa guidelines.
