As a Social Worker at Crossing Rivers Health, you will report to the Inpatient Services Director and provide comprehensive psychosocial support, care coordination, and resource guidance to individuals, families, and groups coping with acute, chronic, or terminal health conditions. Working with the various clinical areas through the organization, this role involves assessing social, emotional, and environmental factors that influence health outcomes and access to care, while delivering supportive interventions, education, and counseling to patients and caregivers. Working collaboratively with the interdisciplinary care team, the Social Worker addresses social determinants of health, assists with care planning and transitions, and supports patient and family decision-making. Requirements:

Education

• Bachelor’s Degree Required, Field of Study: Social Work
• Master’s Degree Preferred, Field of Study: Social Work

Experience

• Minimum of 2 years’ experience in medical social work, case management or related patient support roles: Preferred
• Understanding of healthcare systems, community resources, public benefits, and social service programs
• Familiarity with interdisciplinary team collaboration and care coordination in hospital and outpatient settings

Computer Skills

• Proficient with Microsoft Office and electronic medical record (EMR) systems

Certifications & Licenses

• Active Wisconsin Social Worker license in good standing; required
• Iowa Social Worker license preferred
• Basic Life Support (BLS) certification required
