About This Position

Responsibilities and Duties:
Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input.
Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area.
Ensure timely documentation and compliance with all Paradigm HomeCare policies and procedures.
Collaborate with physicians, nurses, and the care team to support optimal patient outcomes.
Qualifications & Skills:
Master's Degree in Speech-Language Pathology from an accredited university
Active Speech-Language Pathologist license in the State of Texas
Current CPR certification
Valid Texas Driver's License and automobile insurance
Minimum 3 years of experience as a Speech -Language Pathologist, with at least 1 year of recent home health experience (within the last three years)
Knowledge of WellSky EMR software desired
Ideal Candidate Will Be:
Self-Motivated and able to work independently while meeting required deadlines
Clear communicator, both written and oral
Respectful, professional, and courteous in all client, team, and public interactions
Collaborative team player with a patient-first mindset
