Job Description:

St. Gabriel's Hospice and Palliative Care is seeking a compassionate and dedicated Chaplain to join our team in Houston, Texas. As a Chaplain, you will provide spiritual support and guidance to patients and their families during their most challenging times. Your role will be crucial in helping our patients find peace, comfort, and meaning as they navigate the end-of-life journey.

Key Responsibilities:

  • Provide spiritual care and counseling to hospice patients and their families, respecting diverse faith traditions and beliefs.

  • Conduct spiritual assessments and develop individualized care plans in collaboration with the interdisciplinary team.

  • Offer emotional and spiritual support during times of crisis, grief, and loss.

  • Facilitate religious rituals, ceremonies, and sacraments as appropriate and requested by patients and families.

  • Collaborate with community clergy and faith leaders to ensure comprehensive spiritual support.

  • Maintain accurate and timely documentation of all interactions and interventions.

  • Participate in team meetings and case conferences to discuss patient care and progress.

Qualifications:

  • Ordination or endorsement by a recognized religious authority.

  • Master's degree in Divinity, Theology, or a related field preferred.

  • Minimum of 2 years of experience in pastoral care, hospice, palliative care, or a related field preferred.

  • Strong understanding of the spiritual and emotional needs of patients facing terminal illness.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of an interdisciplinary team.

  • Compassionate and empathetic approach to patient care.

  • Flexibility to work on an as-needed basis (PRN).
