Emergency Department Pharmacist at Holyoke Medical Center – Holyoke, Massachusetts

Position Summary:

The successful candidate will provide safe and effective pharmaceutical care in an integrated model, which includes operational and direct patient-centered services in collaboration with physicians, nurses, and other hospital personnel. Activities performed in compliance with the hospitals' mission statement and the policies, procedures, and goals of the department, the hospital and regulatory agencies. This position reports directly to the Pharmacy Clinical Manager and practices under the general direction of the Director of Pharmacy.

Qualifications:

Education:

• Doctor of Pharmacy (PharmD) required

License/Registrations/Certifications:

• Current registration and unencumbered license as a pharmacist with the Massachusetts Board of Registration in Pharmacy – required
• Board Certification through the Board of Pharmacy Specialties as approved by the Clinical Manager or Director of Pharmacy
• ACLS Certification for Clinical Pharmacist - required

Experience:

• Hospital experience preferred
• Demonstrated strong computer skills required
