About This Position

Job Overview:

In this key role, the Pharmacist is responsible for the provision of pharmaceutical care to all patient age groups including geriatric, adult, adolescent, pediatric and neonatal patients,

Qualifications:

Required:

  • Graduate of an ACPE accredited college of Pharmacy, or foreign equivalent educational degree
  • Current New Jersey State license as Registered Pharmacist in good standing
  • Familiarity with all phases of Hospital Pharmacy and a working knowledge of public health matters, as well as, Federal and State laws regulating the professional and hospital practice of Pharmacy
  • Understands the role of the Hospital Pharmacy Department within the hospital and its interrelationships with other departments and functions
  • Promotes positive relations and actively contributes to a positive work environment

Preferred:

  • Bachelor's degree required
  • PharmD preferred
  • Completion of hospital pharmacy residency and/or advanced pharmacy degree preferred

Scheduling Requirements:

  • Evening Shift
  • Per Diem Scheduling

Essential Functions:

  • This individual will prepare, dispense, and monitor both the prescribing and effectiveness of all medications and IV admixtures while exercising considerable independent decision-making ability
  • Utilizes the medication screening process to ensure medication orders are complete, appropriate, within legal parameters and prioritized
  • Evaluates each medication order for formulary status, appropriate dose, dosing interval, allergy, drug/drug interaction, and drug/disease state interaction
  • Oversees preparing of parental admixtures, dispensing, and compounding of medications
  • Contact physicians directly for orders requiring intervention and maintains appropriate entries in the intervention log
  • Notifies nurses of any changes/modifications to medications prescribed
  • Displays self-initiative in anticipating and resolving daily work issues
  • Supervises technical staff to ensure accuracy and compliance with policies and procedures
  • Participates in ongoing education and training related to organization and department specific requirements
  • Properly complies with all national patient safety goals/risk reduction processes and associated policies and procedures as applicable to medication management and the Pharmacy setting
  • Participates in medication error and adverse drug reaction reporting programs
  • Assists with orientation and training of new employees, students, nursing and medical staff
  • Promotes positive relations and actively contributes to a positive work environment
