About This Position
The Patient Engagement Coordinator plays a central role in ensuring that our patients receive consistent, high-quality engagement and are supported in maintaining their ongoing care.
This role will focus on reaching out to existing Accompany Health patients to schedule Annual Visits, follow-up appointments with clinicians, and provide gentle reminders, answer questions, and reinforce the value of our continuous support. Your work will help patients stay engaged with their care teams and build stronger relationships with us over time.
This is a high-volume outbound call role. You will spend the majority of your workday on back-to-back outbound calls, with a target of 135+ calls per day (subject to change as business needs evolve). Success in this role requires the ability to maintain energy, focus, and warmth across a full day of calls — this is not a role for someone who tires of phone work. Your success is measured by your ability to deliver an outstanding patient experience, ensure continuity of care, and maintain accurate records.
- Conduct outbound calls, texts, and other virtual outreach to schedule Annual Visits and follow-up appointments with existing patients
- Maintain a daily outbound call target of 135+ calls, working through structured call queues in our internal CRM tools throughout the day
- Send SMS and message-based outreach to patients to confirm appointments and follow up on scheduling
- Navigate and maintain records across internal CRM systems and spreadsheets simultaneously during and after calls
- Educate patients on the importance of ongoing care and the benefits of completing their annual visits and any necessary follow-up appointments with clinicians
- Handle calls from patients with warmth and professionalism, answering questions and directing them appropriately
- Collaborate closely with clinical care teams to ensure alignment on visit readiness and follow-ups
- Provide an empathetic, respectful, and encouraging patient experience on every call
- Monitor scheduling metrics and contribute to achieving engagement goals, maintaining both call volume targets and service quality
- Act as a liaison between patients and the care team to address basic concerns and questions about services
- Excellent communicator with a warm, patient-first mindset
- Empathetic listener who can connect with a wide variety of individuals, especially seniors and those with complex needs
- Comfortable with high-volume, structured call workflows, energized rather than drained by back-to-back outbound calls throughout the day

- Comfortable working across multiple tools simultaneously, including CRM systems, spreadsheets, and SMS platforms
- High School Diploma/GED or higher
- 2+ years of related experience in a high-volume outbound call environment, preferably in healthcare customer service or healthcare sales/direct marketing
- Excellent patient communication skills, with the ability to quickly build trust and rapport with seniors and other patients
- Strong computer skills and basic knowledge of G-Suite (e.g., Google Docs/Sheets)
- Can handle multiple tasks at once, such as typing notes while talking and working in 2 systems at the same time
- The first stage of the hiring process requires applicants to complete an internet speed test to ensure their connection meets the performance standards necessary for this position
- Experience working in an Electronic Medical Record (EMR) system
- Knowledge of Medicaid and/or Medicare programs and senior services
- Ability to work after hours and weekends if needed
$21 - $21 an hour
