About This Role
This is an opportunity for a mission-driven Nurse Practitioner whos passionate about helping high-risk seniors and frail older adults live the independent lives they deserve. As a key member of our Interdisciplinary Team (IDT), youll provide direct medical care and lead a panel of participantsbuilding trusted relationships and driving care plans that improve outcomes.

Youll meet participants where they areprimarily in the center/clinic, with occasional home visits, virtual care, or other sites as needed. This role is MondayFriday with regular hours, plus shared on-call rotation support. If you thrive in a collaborative, high-growth environment, youll feel right at home here.
What Does Success Look Like In This Role?
  • Be a core clinical voice on the IDT: Partner with the Interdisciplinary Team to complete key assessment sections, contribute to care planning, and support service determination requests.
  • Own and grow your participant panel: Actively build relationships with participants and manage an ongoing panel with continuity, accountability, and heart.
  • Practice thoughtful, safe prescribing: Initiate, evaluate, review, and update medication and treatment orders (including diagnostic studies) with required approval.
  • Coordinate care in real time: Assess, initiate, and coordinate day-to-day care under supervisionkeeping participants stable, supported, and progressing toward goals.
  • Chart with excellence and timeliness: Document participant interactions, assessments, and next steps in the EMR within established center timeframes.
  • Connect participants to the right specialists: Make referrals, request consults, and ensure follow-through with approvalhelping participants navigate complex systems with ease.
  • Deliver comprehensive clinical care: Perform histories and physicals, provide participant education, and support informed decision-making at every step.
  • Stay on top of labs and clinical results: Order, interpret, and follow up on lab tests and resultsclosing loops and acting quickly when needed.
  • Use your hands-on skills where appropriate: Perform selected procedures within scope and support high-quality, efficient care delivery.
  • Spot changes early and escalate appropriately: Recognize abnormalities, respond with clinical judgment, and escalate concerns to ensure participant safety.
  • Collaborate across disciplines: Work closely with behavioral health, pharmacy, and other care partners to ensure wraparound, whole-person care.
  • Meet participants where they are: Evaluate and treat participants in the center, at home, virtually, in SNFs, and other settingsdelivering care across the continuum.
  • Support meaningful goals-of-care conversations: Facilitate advance care planning discussions with participants and caregivers with empathy and clarity.
  • Capture acuity accurately: Document chronic conditions through HCC capture to reflect true clinical complexity and support appropriate care planning.
  • Pitch in when it matters most: Provide on-call coverage as needed, supporting timely guidance and continuity for participants.
What Does An Ideal Candidate Look Like?
  • Lead with mission: Youre energized by serving high-risk seniors and frail older adultsand you bring empathy and purpose to your work every day.
  • Practice with proven NP experience: You bring 2+ years of experience practicing as a Nurse Practitioner, ideally in settings serving medically complex and/or geriatric patients.
  • Bring strong clinical depth: You have hands-on experience caring for complex and older adult populations, with comfort navigating multiple chronic conditions.
  • Communicate with clarity and warmth: Youre a confident communicator who can build trust quickly with participants, caregivers, and teammates.
  • Thrive with autonomy and teamwork: You can lead independently, make sound clinical decisions, and collaborate seamlessly within an interdisciplinary team.
  • Grounded RN foundation: You bring at least 2 years of RN experience supporting medically complex populations prior to becoming a Nurse Practitioner.
  • Licensed and ready to practice: You hold an active, unrestricted California NP license (and California RN license, as applicable) and maintain current CPR certification.
  • Strong educational preparation: Youre a graduate of an accredited School of Nursing and hold a Masters degree from an accredited NP (or PA) program.
  • Credentialed for full-scope practice: You have a Nurse Practitioner certificate with prescriptive authority and stay current on evidence-based standards.
  • Geriatrics-informed and outcomes-driven: You have up-to-date knowledge of chronic care and geriatric best practicesand a commitment to continuous learning.
  • Bilingual skills are a plus:English required; Spanish preferred to better support the communities we serve.
  • Mobile and community-ready: You have a valid California drivers license, reliable transportation, a good driving record, and auto insurance as required by state law.
Pertinent information
  • Consistent weekday schedule: MondayFriday, with regular business hours so you can plan your life outside of work.
  • Team-based on-call support: Participate in an on-call rotation as neededshared coverage that helps ensure continuity of care for participants (with stipend).
  • Dynamic work setting: Primarily based in the center/clinic, with occasional time out in the community (including home visits) to meet participants where they are.
What Are The Working Conditions?
  • Clinic-based, participant-facing role: This position is primarily based in our center/clinic setting, providing direct care to participants who come in for appointments and services.
  • Occasional home visits: From time to time, you may conduct home visits to support participant care needs and continuity outside the center.
  • Indoor and outdoor environments: Work is performed in a clinical/office setting, with periodic time spent outdoors while traveling between locations or during community-based visits.
  • Active, on-your-feet workdays: This role involves frequent walking throughout the center, moving between exam rooms and care areas, and engaging with participants throughout the day.
  • Variable settings across the care continuum: You may support participants in different settings as needed (e.g., center, home, virtual), depending on care plans and operational needs.
  • Exposure to healthcare environments: As a clinical role, there may be regular exposure to typical healthcare conditions (e.g., illness, bodily fluids, medical equipment), following all safety and infection-control protocols.
What Are The Physical Requirements?
  • Frequent standing and walking: Ability to remain on your feet for extended periods and move throughout the center/clinic during the workday.
  • Repetitive movement and fine motor skills: Frequent use of hands for clinical tasks (e.g., examinations, procedures within scope, typing/documenting in the EMR).
  • Bending, reaching, and positioning: Regular bending, reaching, kneeling, and twisting to provide care, access supplies, and work in exam rooms.
  • Lifting and carrying: Ability to lift/carry clinical supplies and equipment and assist with participant mobility as needed (typically up to ~25 lbs independently; more with assistance or proper equipment).
  • Visual and auditory acuity: Ability to read screens/medical records, interpret clinical data, and communicate effectively with participants and team members in a busy care environment.
  • Driving and local travel: Ability to drive for occasional home visits and travel between sites as needed, including getting in/out of a vehicle and carrying basic supplies.
  • Exposure considerations: Ability to work safely around typical clinical exposures (e.g., communicable illnesses, bodily fluids, disinfectants) while following PPE and infection-control protocols.
