CARLTON PLACE JOB DESCRIPTION

POSITION TITLE: Assisted Living (AL) Home Health Aide/Certified Nursing Assistant

REPORTS TO: Carlton Place Director and RN

JOB SUMMARY:

This position is responsible for performing household and personal care services as necessary to maintain the personal comfort and cleanliness of our residents.

DUTIES AND RESPONSIBLITIES:

Duties:

  1. Personal care activities, including but not limited to:

Bathing – full or partial Shampoo/hair care

Skin care/nail care, including foot soaks Oral hygiene/denture care

Shaving Dressing

Ambulation Transfers

Exercises Body positioning

Assistance with dining Administration of

Reminders within resident care plan medications & treatments

Toileting, bedpan & urinal

assistance

  1. Perform household services, including but not limited to:

Meal service & dishes Eating assistance if necessary

Dusting and/or requested

Laundry Replenishing linens

Making beds cleaning windows

Wash mirrors Vacuum

Garbage removal to the dumpster Mopping Floors

Clean & disinfect bathrooms, kitchen & high-touch surfaces

Keeping common areas and resident units clean and orderly

  1. Lead and/or assist in facilitating resident activities as planned by the Director
  2. Assist with bowel and bladder control, bladder training, and incontinence products
  3. Report changes in the resident’s condition and needs to the LPN &/or RN as needed
  4. Complete the appropriate records to document cares given and pertinent observations regarding physical and mental conditions, intake and output, and exercise.
  5. Respond and attend to resident requests promptly or request other team members to assist
  6. Maintain proper hand-washing techniques and infection control
  7. Communicate effectively with residents, families and team members while maintaining confidentiality
  8. Maintain a safe resident environment
  9. Maintain confidentiality; treat residents, family, and team members with respect
  10. Participate in monthly Inservice programs to meet State and site compliance requirements
  11. Taking and recording vital signs: temperature, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency.
  12. Organize and restock carts, resident units, and supply room at the end of the shift
  13. Ensure security of resident rooms
  14. Follow all company safety and security procedures
  15. Report any maintenance issues or safety hazards
  16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
  17. Assist with the assessment, planning, implementing and evaluation of selected routine and non-technical aspects of nursing care under the direct supervision of Registered Nurses (RN`s) and Licensed Practical Nurses (LPNs).
  18. Any and all ‘To Do’ items within EMAR software include resident services, tasks and chores, and are included in job duties.
  19. Perform other related duties and responsibilities as deemed necessary

JOB REQUIREMENTS:

  1. Successful completion of Relias online training courses and competency evaluation and or completion of state approve nurse aide training program.
  2. Successful completion of on-the-job orientation process
  1. Have satisfied the following requirements:
    1. Have demonstrated an accepting and therapeutic attitude toward the sick, disabled, and the elderly
    2. Ability to read, write and follow written and verbal directions in English
    3. Maturity to deal effectively with the demands of the job and deliver good customer service to residents and their families.
    4. Demonstrated the ability to work with little supervision and make appropriate judgments
    5. Demonstrated dependability, tact, and ability to follow orders.
    6. Possess and maintains good physical and mental health, including current TB testing policy
    7. Have US citizenship or evidence of valid alien work permit
  1. Clearance of Minnesota Department of Human Services background study
  2. Physical stamina and mobility including ability to reach, kneel and bend, stand for long periods of time, squat, crouch and work in confined spaces.
  3. Ability to lift, push and pull required load (usually about 50 lbs.)
  4. Ability to tolerate exposure to dust and cleaning chemicals
  5. Must be able to work a schedule that includes rotating holidays, weekends and overtime.
