Job Description

Avery's House is a substance abuse & mental health treatment provider for adolescents, seeking experienced, dependable, and responsible Case Manager. The Case Manager will help the clients with their discharge planning, coordinating with therapists, and online schooling needs. Under general supervision, functions as a liaison between schools, parents, and others to ensure the educational needs of the clients are being met. The clients are Junior High and High School level. Experience with case management and working with adolescents is preferred. This position provides daily school support to those living in Avery's House structured recovery housing.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Case Management needs
  • Discharge planning
  • Coordinating with clinical and medical
  • Communicate with the school and obtain online schooling information.
  • Communicate with the school regarding IEP's.
  • Be present during school work time, monitoring electronic use, and ensuring school work is being done.
  • Reviewing classroom or curricula topics and assignments.
  • Providing students positive and constructive feedback.
  • Building meaningful connections with students from diverse backgrounds.
  • Assisting students with homework, projects, test preparation, papers, research and other academic tasks.
  • Recognizing different learning styles and student preferences.
  • Collaborate with the treatment team.
  • Maintain records and related documentation. Produces reports, correspondence and performs case management duties.
  • Other tasks as assigned.

REQUIREMENTS

  • High School Diploma (Required) Bachelor’s Degree in a behavioral science or related field (Preferred)
  • At least 1 year of full-time work experience with children in a social service setting
  • REQUIRED: Must be minimum 21 years of age.
  • REQUIRED: Must pass background check.
  • REQUIRED: Must pass drug screening.
  • REQUIRED: Must be able to climb up and down stairs, hills, bend and lift 25+lbs
  • Enhanced background clearance with the ID Dept of Health & Welfare (preferred- will be required to obtain)
  • CPR/First Aid certification (preferred- will be required to obtain)
  • Current TB test (will be required to obtain)

Job Type: Full-time

Pay: From $45,000.00 per year
