CASE MGR/CARE COORD 02 at SOUTHERN HIGHLANDS COMMUNITY MENTAL HEALTH C – Mullens, West Virginia

We are seeking a compassionate, detail-oriented Case Manager / Care Coordinator – Level 2 to join our Outpatient team. In this role, you will play a vital part in helping clients navigate and access essential behavioral, medical, social, educational, and health-related services.

The Case Manager / Care Coordinator Level 2 assists clients in identifying needs, accessing resources, and achieving treatment goals through coordinated, person- and family-centered care.

This role includes:

• Coordinating behavioral, medical, social, and educational services

• Advocating on behalf of clients and families

• Developing and implementing individualized treatment plans

• Monitoring progress and adjusting services as needed

• Collaborating with interdisciplinary providers and community partners

• Maintaining accurate and timely documentation

Candidates must meet one of the following qualifications:

• Bachelor’s or Master’s degree in a human services-related field (in accordance with the West Virginia Board of Social Work), OR

• Licensed Psychologist (Master’s or Doctoral degree from an accredited program), OR

• Licensed Registered Nurse (RN)

• Strong organizational and communication skills

• Ability to work collaboratively within a multidisciplinary team

• Commitment to ethical practice and client-centered care

• Passion for serving individuals and families in outpatient settings
