As a Registered Practical Nurse, you’ll collaborate with clients within our housing departments, to assess and advocate for the supports they need to achieve physical and mental wellness. Our Nursing Team works with community and hospital partners to provide excellent collaborative care, focusing on social determinants of health. This role performs a variety of functions including administration of psychotropic injections, health teaching, medication reconciliation, coordinating appointments, providing diabetes care and lifestyle supports to afford clients the ability to achieve success with their recovery goals.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $30.01 per hour (Level 1 of the Regulated Direct Service Pay Grid);
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $37.26 per hour
    • Internal applicants will be placed on the grid based on their seniority
  • When you'll work – 70 hours over a two-week period; Monday-Friday 9am-4:30pm with a 30-minute unpaid lunch; Option of a four-day work week (Monday-Thursday, 8:00am -5:15pm) including a 30 min unpaid lunch
  • What your benefits will be - 9% in lieu of benefits
  • Your pension plan – Optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP)


HOW YOU WILL BE MAKING A DIFFERENCE:
Nursing Assessments:

  • Conducts physical and psychiatric assessments and makes appropriate referrals to community physicians and resources
  • Assesses and documents medication effectiveness, complications, and side effects
  • Identifies, develops, and maintains strong links with a wide range of community partners/resources
  • Organizes, coordinates, and administers depot injections
  • Reviews and audits completion of Medication Administration Records (MARs) completed by staff within the home, completing incident reports and taking appropriate measures in response to medication errors to ensure health and safety of affected tenants

Health Promotion:

  • Assists participants in making informed choices about health measures such as medications, immunization, palliative care, disease management, etc.
  • Provides health teaching by giving feedback, explanations, and rationale for diagnostic tests, procedures, and treatments
  • Supports the education and Medication Management policy and procedure training of CHO private home staff
  • Assists participants in taking responsibility for and managing their health needs according to available resources and personal skills
  • Identifies and seeks to address the root causes of illness and disease
  • Assists participants in making healthier choices
  • Collaborates with community agencies to maintain coordination in the treatment process
  • Advocates for participants to obtain psychiatrists, family doctors, dentists, etc.

Crisis Intervention

  • Identifies high risk participants and assess the need for crisis intervention, prioritizing the safety of participants
  • Performs work in a calm, competent, and efficient manner in emergency situations while acknowledging the participant’s need for privacy and dignity
  • Utilizes harm reduction principles to identify, reduce, and remove risk factor
  • Takes action as needed to respond to urgent health concerns
  • Documents and reviews suicidal behaviors, thoughts, and attempts and refers to appropriate community resources as necessary


WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 5 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.


We require the following qualifications:

  • Diploma in Nursing as a Registered Practical Nurse (RPN) (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Current Registration from the College of Nurses of Ontario
  • Valid Driver’s Licence, access to a vehicle and minimum $2 million liability insurance to carry out job duties
  • Current CPR Certificate
  • Demonstrated ability to attend work regularly
