We are currently recruiting for Full Time / Part Time CHHA's on 7-3 and 3-11. Per Diem Opportunities available on 11-7 shift.
About the Role:
The Care Partner CHHA (Certified Home Health Aide) plays a vital role in providing compassionate, personalized care to patients in their homes across the United States. This position focuses on assisting individuals with daily living activities, ensuring their comfort, safety, and well-being while promoting independence. The Care Partner CHHA collaborates closely with healthcare professionals to implement care plans tailored to each patient's unique needs. Through attentive observation and communication, the role contributes to monitoring patients' health status and reporting any changes promptly. Ultimately, this position aims to enhance the quality of life for patients by delivering respectful, empathetic, and professional home health care services.
Minimum Qualifications:
- Valid Certified Home Health Aide (CHHA) certification recognized in the United States.
- High school diploma or equivalent.
- Ability to pass background checks and health screenings as required by state regulations.
- Demonstrated ability to perform physical tasks associated with patient care.
- Strong communication skills and basic literacy to document care activities accurately.
Preferred Qualifications:
- Previous experience working as a Home Health Aide or in a similar caregiving role.
- CPR and First Aid certification.
- Familiarity with electronic health record (EHR) systems.
- Additional training or certification in specialized care areas such as dementia or palliative care.
- Bilingual abilities to effectively communicate with diverse patient populations.
Responsibilities:
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
- Provide basic health-related services including monitoring vital signs and reporting any changes to nursing staff.
- Support patients with medication reminders and adherence under the supervision of healthcare professionals.
- Maintain a clean, safe, and comfortable environment for patients within their homes.
- Document care provided and communicate effectively with the healthcare team to ensure continuity of care.
- Offer emotional support and companionship to patients to promote mental and emotional well-being.
- Adhere to all safety protocols, infection control standards, and patient confidentiality regulations.

The Care Partner CHHA utilizes strong interpersonal and communication skills daily to build trust and rapport with patients and their families, ensuring clear understanding of care plans and patient needs. Physical stamina and dexterity are essential for assisting with mobility and personal care tasks safely and effectively. Attention to detail is critical when monitoring patient conditions and documenting care activities to maintain accurate health records. Problem-solving skills help in adapting to changing patient needs and responding appropriately to emergencies or unexpected situations. Additionally, empathy and patience are fundamental in providing emotional support and fostering a positive caregiving environment.
