About This Position

RiseWell Community Services is seeking a Full Time Case Managers to play a pivotal role on one the Governor’s offices newly launched innovative Safe Options Support (SOS) team that that will provide comprehensive care to street homeless or subway dwelling individuals in Brooklyn & Queens.

In this role, you will work as a member of a multi-disciplinary team to assess health and social needs of the homeless population. This includes but is not limited to assisting the team with: community outreach/canvassing, specifically on the streets and subways throughout the Brooklyn/Queens area, coordinating participant’s referral with appropriate overnight facilities, accompany, advocacy, and assist on ensuring warm hand offs and follow-ups are maintain with SOS day teams, etc. Maintaining collaborative relationships with community partners including the Department of Homeless Services, MTA, local law enforcement, community hospitals, existing outreach teams and housing drop-in shelter.

QUALIFICATIONS:
A minimum of 4 years of experience in case management. Bachelor's degree in Human Services preferred with previous case management experience. Clean valid NY State Driver’s License.Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
