Physical Therapist (Home Health) at Vanura Homehealth Services Inc – Rancho Cucamonga, California

Essential Functions

Responsible for providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.
Responsible in gathering background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients; and in determining the level of patient's physical and psychosocial functioning.
Responsible in developing and revising the plan of care in consultation with the physician and other care-team members.
Responsible in preparing clinical progress notes.

Qualifications:

Currently licensed by the California Board of Physical Therapy
Minimum of one (1) year in the acute care setting.
Preferably with one (1) year Home Health experience.
