About This Position
Registered Dietitians foster trustworthy relationships with patients and their families, employing dietary education and planning to enhance the quality of life for their patients.
Responsibilities encompass conducting patient assessments, developing tailored nutritional plans, and offering ongoing nutritional support. The ideal candidate must hold registration with the Commission on Dietetic Registration and possess current state licensure or a minimum of one year of clinical nutrition experience.
Experience with patients diagnosed with End-Stage Renal Disease (ESRD) is preferred.
**Essential Functions**
The essential functions include, but are not limited to, the following:
1. Adherence to all Company policies and procedures, as well as federal and state regulations, including OSHA and other applicable agency guidelines for the role.
2. Management of patient information with discretion and sensitivity to ensure HIPAA compliance.
3. Utilization of professional communication to promote positive relations with patients, family members, physicians, colleagues, outside agencies, and service providers.
4. Assurance that patients receive appropriate care in relation to renal diets and medications within Long-Term Care Facilities.
5. Provision of comprehensive nutritional assessments for dialysis patients, ongoing instruction and counseling, monitoring adherence to dietary plans, supplying educational materials, and reviewing monthly laboratory results.
6. Assistance with the acquisition of Intradialytic Parenteral Nutrition (IDPN), nutritional resources, and protocols.
7. Engagement as a member of the Interdisciplinary Team (IDT) to complete Comprehensive Assessments and Plans of Care for all patients, including participation in care plan meetings and ensuring coordination with other service providers as necessary.
8. Participation in the Quality Assessment Performance Improvement (QAPI) team, contributing to the facility’s Continuous Quality Improvement process, monitoring clinical indicators, attending meetings, and assisting with action plans in accordance with QAPI Program requirements.
9. Involvement in national and state renal nutritional services programs while staying informed about developments in the field.
10. Review of monthly laboratory results for each patient, making necessary adjustments to medications and dietary orders in collaboration with the attending physician or relevant protocols and complying with state-specific regulations. Assistance with the management of medications per protocols (e.g., Vitamin D analogs, calcimimetics, phosphorus binders), as well as obtaining Intradialytic Parenteral Nutrition (IPN) and other nutritional supplements for patients.
11. Referral of patients to nutritional resources, including financial assistance, community resources, or in-home support, and facilitating access to nutritional supplements as necessary.
12. Execution of job responsibilities in accordance with all state and licensing requirements.
13. Collaboration on additional projects and assignments as requested by Clinic Leadership.
Qualifications
- Registration with the Academy of Nutrition and Dietetics through the Commission on Dietetics Registry is required.
- Current state licensure, if mandated by the state, along with compliance with all relevant state and licensing requirements, is required.
- A minimum of one year of professional experience in clinical nutrition as a Registered Dietitian is required.
- Membership in the American Dietetic Association is strongly preferred. - Prior experience in nutrition related to patients with ESRD is preferred.
- The candidate must maintain registration and fulfill continuing education requirements as stipulated by the American Dietetic Association.
- Candidates should possess the ability to read and interpret safety rules, operating instructions, and procedural manuals.
- Proficient communication skills are essential for effective interaction with clients and colleagues.
**Certification Requirements**
- Basic Life Support (BLS) certification is required.
