Licensed Practical Nurse (LPN/LVN)

What You’ll Do
As an LPN/LVN, you will work under the supervision of a Registered Nurse to provide quality nursing care and health support for individuals receiving services in residential and community-based settings.
Responsibilities Include:
- Administer medications and treatments
- Monitor health conditions and report changes to clinical leadership
- Assist individuals with activities of daily living (ADLs)
- Maintain accurate clinical documentation and medication records
- Support individualized healthcare and behavioral support plans
- Promote a safe, respectful, and person-centered environment
- Participate in interdisciplinary team collaboration
- Respond to medical emergencies as needed
- Reinforce infection control and basic healthcare practices with support staff.

Work Environment
This position involves working in residential homes, community settings, and healthcare environments. Candidates should be comfortable with frequent movement, travel within the assigned area, and direct client interaction.

Requirements:What We’re Looking For
Required Qualifications- Active LPN/LVN license
- Graduate of an accredited LPN/LVN program
- Current BLS certification
- Valid driver’s license and reliable transportation
- Experience working with individuals with IDD/DD populations
- Long-term care or community-based nursing experience
- Experience with electronic health records (EHR)
