Description
The Partnerships Manager is responsible for developing, nurturing, and supporting a broad and diverse portfolio of referral and community partners across Sierra Health & Wellness’s service regions. This role focuses on relationship-building, partner education, community engagement, and referral support ensuring partners have a seamless, positive experience. This role serves as a collaborative liaison, ensuring partners feel supported and informed while helping the organization expand access to care.
Essential Functions
Build and strengthen relationships with diverse referral sources including health systems, outpatient providers, schools, EAPs, county programs, and community organizations.
Conduct outreach meetings, presentations, and educational sessions to increase awareness of Sierra’s programs and referral pathways.
Identify new partnership opportunities and expand presence in assigned territories.
Represent the organization at community events, trainings, conferences, and networking functions.
Maintain consistent communication with assigned partners through in-person visits, calls, emails, and virtual meetings.
Provide timely updates and assistance to partners regarding referrals, patient transitions, and service options.
Document all partner interactions and follow-up tasks in the CRM daily.
Monitor partner satisfaction and identify opportunities to strengthen the relationship.
Coordinate with Admissions, Clinical, Marketing, and Leadership teams to address partner needs and resolve issues.
Report field trends, opportunities, challenges, and partner insights to leadership.
Support organizational initiatives by providing real-time feedback from the field.
Maintain accurate CRM documentation for all activities and partner interactions.
Submit weekly and monthly reports summarizing activity levels, trends, wins, and challenges.
Track referral trends and engagement patterns to guide strategic planning.

Work Environment
Office and healthcare environments with regular travel to partner locations.
Working indoors primarily with occasional outdoor or event-based activities.
May involve working in close proximity to others and occasional interruptions
Physical Demands
Prolonged periods of driving, sitting, presenting, or computer work.
Must be able to lift up to 15 pounds occasionally.
Travel Required
Frequent local and regional travel; occasional out-of-state travel for conferences or organizational needs.
Required education and experience
High School diploma or GED required.
1–3 years of experience in healthcare outreach, partnerships, business development, or community engagement.
Experience using CRM systems and Microsoft Office Suite.
Preferred education and experience
Bachelor’s degree in Business, Marketing, Communications, Social Services, Public Health, or related field.
3–5 years of experience in healthcare outreach, provider relations, or referral development.
Prior experience in behavioral health or addiction treatment.
Experience working with hospitals, outpatient providers, county agencies, or community programs.
Work authorization/security clearance requirements:
Valid Driver’s License.
