The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

We are seeking a bilingual Primary Care Provider—Physician (MD or DO), Nurse Practitioner, or Physician Assistant—to join our team at MNHC’s Homeless Resource Center.

Primary Objective:
Under the supervision of the Chief Medical Officer, the primary care provider at the Homeless Resource Center delivers preventive and acute care, substance use and mental health counseling, integrated with case management and support services in the Drop-In Center.
  • Conduct medical assessments, develop treatment plans, and coordinate referrals.
  • Collaborate with a multidisciplinary team, including nurses, medical assistants, behavioral health providers, substance use counselor, and care managers.
  • Maintain accurate documentation in the Electronic Health Record to ensure continuity of care.
  • Participate in quality improvement initiatives to enhance patient outcomes.
  • Advocate for health equity and culturally responsive care.
  • (For Physicians) Supervise and facilitate consultations for NP's and/or PA's.
Qualifications
  • California-licensed MD or DO with Board Certification in Family Medicine or Internal Medicine, Adult/Geriatric NP, Family NP, or PA with adult medicine training
  • Fluent in Spanish (required).
  • At least 3 years of post-graduate clinical experience in primary care (can include residency programs)
  • Experience with underserved populations, including the unhoused individual
Compensation
  • Physician (MD/DO): $234,600 - 259,590 (FT)
  • Nurse Practitioner (FNP): $133,008 – $146,370 (FT)

As a condition of employment, all candidates are required to meet specific immunization standards.
