Ready to make a meaningful impact from the very first patient touchpoint? Singlepoint Healthcare is looking for an Intake Nurse Specialist to help coordinate safe, smooth, and compassionate home infusion care for patients with inflammatory diseases.
What You’ll Do 💙✔️ Assess patients for home infusion therapy
✔️ Coordinate care plans, supplies, and delivery schedules
✔️ Partner with home health agencies, referral sources, and pharmacy teams
✔️ Support patient and caregiver education
✔️ Monitor progress, therapy compliance, and care needs
✔️ Keep clinical documentation accurate and timely
✅ RN or LPN license in the state of practice
✅ Associate’s degree
✅ At least 1 year of nursing experience in home health or infusion therapy
✅ Knowledge of infusion techniques, home care protocols, and patient education
✅ Strong communication, organization, and patient-first mindset
