POSITION SUMMARY:
The Pediatric Triage Nurse (LPN or RN) is a vital clinical resource and the first point of contact for families seeking care and guidance. In this role, you’ll provide both phone and in-office triage, supporting parents with everything from sick visit concerns and medical advice to prescription refills, referrals, and care coordination. Working closely with providers and the care team, you’ll help ensure timely, accurate, and compassionate care for every patient. This position also plays a key role in patient and family education, maintaining thorough clinical documentation, and supporting seamless continuity of care across the practice.
Schedule: Full-time (40 hours/week), on-site only.
ESSENTIAL RESPONSIBILITIES: To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation.
- Serve as the primary point of contact for patient health inquiries, providing timely and professional guidance via telephone and in-office interactions
- Perform clinical triage by assessing patient symptoms, determining urgency, and directing patients to the appropriate level of care
- Conduct patient assessments and implement appropriate nursing interventions within scope of practice
- Communicate effectively with patients, providers, and staff in a professional and courteous manner
- Communicate test results, provider recommendations, and other clinical information clearly and compassionately
- Return messages and follow up on patient requests via phone or patient portal in a timely manner
- Coordinate and schedule ancillary services, including labs, imaging, and specialist referrals, per provider orders
- Assist with scheduling sick visits, consults, and other appointment types based on clinical need
- Collaborate with providers and interdisciplinary team members to ensure continuity and quality of care
- Facilitate communication across departments to support seamless patient care delivery
- Provide patient and family education, counseling, and clear instructions regarding care plans
- Accurately document all patient interactions, assessments, and care coordination activities in the electronic health record (EHR)
- Update patient charts during visits under provider direction, including medical histories, exams, and treatment plans using appropriate medical terminology
- Provide patients with documentation and instructions related to their care plan
- Maintain working knowledge of programs such as Vaccines for Children (VFC)
OTHER RESPONSIBILITIES:
- Maintain strict confidentiality; adheres to all OSHA, HIPAA, and other federal, state, and local guidelines/regulations.
- Demonstrate dedication to the mission, vision, values, and goals of the organization.
- Regular and reliable attendance including attending monthly staff meetings.
- Perform other duties as assigned
- High school diploma or GED required
- Completion of an accredited nursing education program required
- Current, active, and unencumbered licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of practice required
- Minimum of two (2) years of clinical nursing experience required
- Prior triage experience in a pediatric or ambulatory care setting
- Demonstrated pediatric clinical experience required
- Current Basic Life Support (BLS) certification required and maintained
PREFERRED QUALIFICATIONS:
- Bachelor of Science in Nursing (BSN)
- Pediatric Advanced Life Support (PALS) certification
- Bilingual proficiency in Spanish
KNOWLEDGE, SKILLS, & ABILITIES:
- Knowledge of pediatric nursing principles, practices, and standards of care
- Ability to perform clinical triage and assess patient needs with appropriate prioritization
- Demonstrated clinical judgment, critical thinking, and professionalism
- Effective verbal and written communication skills with patients, families, and interdisciplinary team members, including diverse populations
- Strong interpersonal skills with the ability to engage parents, caregivers, and pediatric patients in high-stress situations with professionalism, empathy, and compassion
- Knowledge of current medical terminology sufficient to communicate with providers, staff, and patients
- Ability to maintain accurate, timely, and compliant clinical documentation in the electronic health record (EHR)
- Strong organizational skills with attention to detail, ability to manage multiple tasks and priorities, and perform under pressure
- Ability to work independently with minimal supervision and exercise appropriate initiative
- Knowledge of applicable regulatory, accreditation, and patient safety standards
- Ability to work effectively in a fast-paced, dynamic clinical environment with changing priorities
