MAJOR FUNCTION: The Health Solutions Coordinator plays a central role in planning, coordinating, and executing school-based medical and dental programs, as well as community outreach through the Mobile Medical Unit. This position ensures that students have access to preventive and routine healthcare services by organizing on-site visits, managing consent and enrollment processes, and coordinating schedules with school personnel and CompleteCare.
The coordinator works closely with school administrators, nurses, and staff to identify student needs and facilitate services such as well-child visits, sports physicals, immunizations, and dental care. The role includes managing all logistical components, including student rosters, appointment scheduling, parent/guardian communication, and ensuring proper documentation.
In the community, the coordinator supports and promotes the Mobile Medical Unit by organizing clinical events, identifying service locations, and engaging with community partners to expand access to care. This includes outreach to underserved populations and coordination of mobile clinic schedules.
The coordinator serves as a key liaison between families, schools, and community providers, ensuring clear communication. This role requires strong organizational skills, attention to detail, and the ability to adapt quickly in dynamic environments while maintaining a high standard of patient-centered care.
ESSENTIAL RESPONSIBILITES:
- Promote awareness and enrollment in the Health Solutions Program by providing information and resources to students, parents/guardians, and community resources.
- · Coordinate student enrollment, including consent forms, insurance verification, well visits, sports physicals, and dental services.
- · Collaborate with school nurses to identify students participating in daily trips and services
- · Coordinate transportation and chaperoning with the Outreach Department to ensure smooth operations
- · Promote and support Mobile Medical Unit clinics within the community
- · Serve as a chaperone for medical and dental service visits
- · Identify students requiring follow-up care
- · Perform insurance verification, referral processing, and document management (scanning and EMR updates)
- · Register new patients in the EMR system and maintain accurate records and spreadsheets
- · Participating in school functions and committees as needed
- · Support and host community outreach events
- · Conduct visits to migrant and farm camps (May–October) to provide health screenings
- · Perform other duties as assigned

Requirements:QUALIFICATIONS:
- · High School Diploma or equivalent required
- · Experience working with children and community populations preferred
- · Strong communication skills (verbal, written, and listening)
- · Ability to effectively collaborate with school staff and administrators
- · Proficiency in Microsoft Excel
- · Strong organizational skills and ability to multitask
- · Ability to work both independently and collaboratively
- · Willingness to attend outreach events
- · Bilingual preferred
- · CPR certification required
- · Medical Assistant certification license preferred
- · Experience working with electronic Medical Records
Requirements:
- · Ability to lift up to 15 pounds
- · Valid New Jersey Driver’s License and reliable transportation
- · Must be eligible for inclusion on the company insurance policy
- · Must pass fingerprinting and clearance through the NJ Department of Education
- · Ability to obtain clearance to work within the Cumberland County Jail
- · Availability to work evenings and weekends as needed
