POSITION SUMMARY:

Under the direction of the RN Case Manager, provides nursing services/ patient care activities and accommodation services in accordance with standards, regulatory guidelines, and licensure requirements. Follows the plan of care while providing services. Communicates with Manger changes and collaborates with patient/family member/physician or Nurse Practitioner as needed or directed.

Minimal Education: Unencumbered Virginia license as a Licensed Practical Nurse. Graduate of an accredited school of nursing. Minimal Experience: One year of nursing experience, with Hospice and/or Home Health.
