About This Position
Part-time Advanced Practice Primary Care Provider (Up to 20 Hours Per Week)
Family Practice: preventive care, women's health, and annual wellness checks; manage acute needs, chronic illnesses, and complex cases in a team-based model.
This role expands access to gender concordant care for patients who request a provider of a specific gender for women’s health and other sensitive examinations. The clinician in this position will provide full-spectrum preventive care for all ages and genders, with dedicated focus on women’s health.
This is an excellent opportunity to deliver meaningful care within an organization that places the patient at the center. For a portion of our patients, this includes care in an Integrated Team-based Care Model with Primary Care, Behavioral Health and Dental services.
APCs at Bridge Clinic:
- Build trust and collaborate with patients during 30-, 45-, or 60-minute appointments.
- Collaborate with our Family Practice and Addiction Medicine Physician; Adult, Pediatric, and Psychiatric Mental Health Advanced Practice Clinicians; Psychologist and Psychotherapists.
- Co-manage chronic illnesses, complex conditions, and mental and oral health needs in our team-based environment.
- Work closely with our Psychologist, Psychotherapists, and Psychiatric Mental Health Nurse Practitioner to co-manager mental health and substance use disorder needs.
- Partner with our Medical-Dental Integration Hygienist, who is embedded in the primary care visit and located within our integrated care pod, to provide early dental prevention and intervention services.
- Connect patients with our Integrated Care Coordinator to address basic needs and other social drivers of health.
- Contribute to continuous improvement of workflows, quality outcomes, and the overall patient experience.

Required Qualifications:
- Master's degree from an accredited nurse practitioner or physician assistant program.
- Minimum of 2 years of experience in family practice or women's health.
- Board certification
- Current, unrestricted Wisconsin license in good standing
- Current DEA registration
- Current BLS certification
Preferred Qualifications:
- Multilingual skills
- Experience working in a nonprofit, Community Health Center, or Federally Qualified Health Center settings.
- Experience working in a Free clinic, FQHC look-alike clinic, or Tribal health clinic
- Experience with or interest in substance use treatment and medication-assisted treatment for opioid addiction.
