RN On-Call Phone Triage at Hospice of Holland Inc – Holland, Michigan

Current Open Positions:

• Weekend Days Part-Time On-Call RN: (every other Saturday and Sunday daytime) 8:00 a.m. to 8:00 p.m. Please note that applicants must be locally domiciled within Hospice of Holland's service area (Muskegon, Ottawa, Allegan, Kent, or Van Buren counties) and be able to access the administrative office in Holland, Michigan, if necessary.

Position Summary:

The Registered Nurse Phone Triage role provides skilled nursing care for Hospice of Holland patients. Based on incoming calls, he/she assesses the needs of the patient/family unit and provides appropriate medical advice to callers based on existing orders. When necessary he/she collaborates with a Hospice of Holland provider to obtain new orders to assign with immediate symptom management. Communicates outcome of calls to the interdisciplinary plan of care.

Position Responsibilities Include (but not limited to):

• Based on contact with patient, family or facility staff, completes nursing assessment to meet immediate needs. Updates plan of care, including the patient, family and facility staff in the planning process.
• Consults with physician for appropriate orders and communicates patient’s condition changes and the needs for medical intervention.
• Initiates appropriate interventions by way of phone support or referral to RNCM for in person interventions.
• Educates the patient/family as appropriate per assessment and care plan, and encourages their participation.
• Records accurate and timely documentation of patient and family contacts and updates the interdisciplinary team.
• Coordinates Hospice nursing care when outside agency involvement is required.
• Ensures necessary medical equipment, supplies, and services are available.
• Communicates as needed with appropriate external services providers to insure patient/family needs are effectively being met.

Position Qualifications Include:

• Graduate of an accredited school of nursing and current licensure in State of Michigan.
• Has two years of recent clinical experience. Home care or Hospice experience is desired.
