Home Health Physical Therapist - Telehealth at Sierra Home Health and Hospice – Las Cruces, New Mexico

About the Role:
Sierra Home Health is seeking a passionate and patient-focused Physical Therapist to join our dedicated team in Truth or Consequences, NM. You'll work independently with the support of a tight-knit clinical team, delivering personalized care in a community that values relationship-driven healthcare.

Key Responsibilities:

• Deliver physician-ordered physical therapy services in patients' homes
  
• Evaluate patient function and create individualized care plans
  
• Conduct initial assessments, reassessments, and plan of care revisions every 30 days
  
• Supervise Physical Therapy Assistants and Home Health Aides as needed
  
• Educate patients and caregivers on exercises and home care routines
  
• Collaborate with the care team and physicians on treatment adjustments
  
• Prepare timely and accurate clinical documentation, including OASIS
  
• Contribute to quality initiatives, in-services, and patient care conferences

What We’re Looking For:

• Current licensure as a Physical Therapist in New Mexico
  
• Graduate of an accredited Physical Therapy program
  
• Strong assessment and care planning skills
  
• Ability to work independently and manage a home-based caseload
  
• Excellent communication with patients, families, and team members
  
• Valid driver’s license and reliable transportation
  
• Prior home health experience preferred, but new grads are welcome!
