Position Summary
-
The Population Management Navigator assumes responsibility and accountability for the assessment, diagnosis, and management of all eligible SJCHS co-workers and dependents seen in the Wellpath coworker clinic. Ensures all member including SJCHS coworkers and Contracted companies are referred to the appropriate SJCHS Disease Management program, Occupational Health, Primary Care provider, Specialty provider or health/social services when appropriate. In collaboration with program managers, monitors compliance within programs. Assists with contracted companies as needed.

Core Job Functions
-
Obtains medical information and provides physical examinations according to standards and scope of care.

Refers all members including SJCHS coworkers and contracted companies to the appropriate SJCHS Disease Management program, Occupational Health, Primary Care provider, Specialty provider or health/social services when appropriate.

Documents progress notes, to include transmitting that information orally or through notation in the patient’s medical record to the supervising physician for review, and confer with attending physicians on patients presenting with more complex problems.
- Prescribe and manage medications according to standards. Order required labs and other medical studies according to standards. Coordinates care with attending physician according to standards.
- Performs screenings according to standards. Coordinates and monitors health goals according to standards. Provides additional guidance on treatment alternatives and is available by phone for urgent needs and care guidance on a limited basis.
- Adheres to prescription drug or device order guidelines in any health care setting in accordance with regulations. Compliant with all standards of practitioner requirement.
