JOB SUMMARY:

The Wound Care Provider (MD/DO) provides specialized wound care services to patients in home settings, nursing homes, and assisted living facilities. This role involves direct, onsite care for patients with chronic wounds, pressure injuries, surgical wounds, and other complex skin conditions. The Physician is responsible for performing comprehensive assessments, developing and managing individualized care plans, educating patients and caregivers, and working collaboratively with healthcare professionals across various settings to achieve optimal healing outcomes. The Physician also inputs and manages medical orders, updates patient charts in a timely and accurate manner, and ensures all documentation meets regulatory and organizational standards.

Essential Functions:

  • Regularly reassesses current wound each visit and report any concerns to Director.
  • Provides Wound Care services, treatments and preventive procedures.
  • Initiates procedures appropriate for the patient’s wound care.
  • Supervises and trains other nursing service personnel.
  • Performs specific wound care procedures as dictated by provider.
  • Attends team conferences and daily stand downs to give report as necessary.
  • Maintains documentation of all patient contact including visits, calls, emails, texts, etc.
  • Follows the policies and procedures of Dynamic Wound Care. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Always communicate with the supervisor if unable to meet a patient’s needs.
  • Coordinates the implementation of treatment plan with other providers involved in the patients care.
  • Organizes work schedule to be efficient and manage time.
  • Complies with agency infection control policies and protocols.
  • Assist with orientation, teaching and training as requested.
  • In-depth knowledge of advanced wound care, wound healing processes, and infection management.
  • Strong assessment, diagnostic, and procedural skills in field settings.
  • Excellent interpersonal and communication skills, with the ability to educate patients and collaborate with caregivers and staff.
  • Ability to adapt to various home and facility environments with professionalism and flexibility.
  • Other duties as assigned by Director.

Requirements:

QUALIFICATIONS:

Educational/Degree: Master’s or Doctoral degree in Nursing, specializing as a Nurse Practitioner.

Training/Licensure: Currently licensed as a MD/DO to practice in the state.

Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Acceptance of and adaptability to different social, racial, cultural and religious modes. Complete Wound Care Certification within 6 months of hire date.

Experience: Minimum 2 years of experience as an NP, PA, MD, DO.

Active patient contact within past three years, preferred.

Physical Requirements:

Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.

Mental Requirements:

Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:

This is a field-based position requiring travel to patients’ homes, nursing homes, and assisted living facilities within the designated region. The role may involve exposure to biohazardous and infectious materials and requires adherence to safety protocols.

Transportation:

Must have a current valid driver’s license, auto liability insurance and reliable transportation.
