About the Role

As a Hospice Admissions RN at Touchstone Hospice, you are often the first steady presence families encounter during one of the hardest moments of their lives. You guide conversations that require clarity, compassion, and confidence — helping patients and families understand hospice, make informed decisions, and transition into care with dignity and trust.

This role is ideal for an experienced hospice admissions nurse who values autonomy, meaningful patient connection, and a team culture built on respect and collaboration. Your clinical judgment, emotional steadiness, and ability to explain hospice with calm assurance will directly shape each family’s first experience with our care.

Key Responsibilities
  • Conduct comprehensive hospice admission assessments, including physical, emotional, psychosocial, and spiritual needs
  • Evaluate hospice eligibility based on patient condition, prognosis, and CMS criteria
  • Educate patients and families on hospice philosophy, goals of care, and available services with clarity and empathy
  • Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary team
  • Coordinate smooth, timely transitions into hospice care with accurate and complete documentation
  • Serve as a clinical liaison with referral sources, facilities, and internal hospice staff
  • Advocate for patient preferences, values, and goals throughout the admission process
Requirements: What We’re Looking For
  • Current, unencumbered RN license in Texas
  • Prior hospice admissions experience required
  • Strong clinical judgment and confidence explaining hospice to patients and families
  • Emotional steadiness and empathy in complex, high-stakes conversations
  • Ability to work independently in the field while staying connected to the team
  • Excellent communication skills across diverse cultural, spiritual, and emotional needs
  • CPR certification, reliable transportation, valid driver’s license, and auto insurance
