The 245D DSP provides person-centered supports to individuals with disabilities and older adults in their homes and communities, following Minnesota’s HCBS standards. DSPs implement services consistent with each person’s Coordinated Services and Support Plan (CSSP) and CSSP Addendum, ensuring health, safety, dignity, and rights.
Key Responsibilities
· Deliver basic or intensive 245D services (e.g., personal support, night supervision, ILS/SILS, in-home family support, residential supports, employment/day services) as authorized in the CSSP/CSSP Addendum.
· Support ADLs/IADLs, skill development, community integration, and positive behavior strategies in line with service plans and provider policies.
· Uphold service recipient rights, protection standards, incident reporting, and data privacy requirements per 245D and agency policy.
· Accurately complete documentation and maintain compliance with DHS requirements.
Requirements:Required Qualifications
· Must be at least 16 years old and meet DHS staff qualification and competency standards under Minn. Stat. 245D.09.
· Able to pass NETStudy 2.0 DHS background study and not be listed on OIG exclusions.
· Completion of 245D orientation/annual training (person-centered practices, rights, mandated reporting, OSHA/universal precautions, HIPAA, anything else as required by client specific plan.
· Reliable transportation and the ability to work in homes/community settings and meet physical requirements of the role.
Training & Supervision Requirements
· Ongoing supervision and competency assessed by knowledge testing or observed skill per 245D, annual performance evaluations.
· CSSP/CSSP Addendum guidance provided within 15 days of service initiation and updated as needed.
Physical & Work Environment
- Work is performed in clients’ homes and community settings.
- May involve supporting individuals with complex medical, behavioral, or physical needs.
- Must be able to stand, lift, transfer, and assist continuously during shifts.
