Care Coordination RN – Guide Patients Through Their Mental Health Recovery Journey

Position: Care Coordination RN

Location: Hays, Kansas

Pay: starting at $57,061 (based on experience) | $500 SIGN-ON BONUS!

Job Type: Full-Time | In-Person

Schedule: typical hours Monday-Friday 8am-5pm

At High Plains Mental Health Center, our Care Coordination RN will play a critical role in helping individuals successfully navigate these transitions. You’ll work alongside hospitals, psychiatric facilities, providers, and community partners to ensure patients receive safe, continuous, and well-coordinated care.

What You’ll Do:

  • Help patients successfully transition from inpatient psychiatric units, hospitals, and residential programs back into community care
  • Work closely with providers, nurses, and community partners to keep treatment plans aligned and effective
  • Improve patient safety by reviewing medications and coordinating follow-up care
  • Educate and support patients so they understand their care plan and feel confident moving forward
  • Build relationships across healthcare systems that strengthen mental health care in our region

Requirements:
  • Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing. Preferred- Bachelor of Science in Nursing (BSN).
  • Preferred: experience in psychiatric/mental health nursing, or physician office or outpatient practice.
  • Preferred: experience in care coordination or case management.
  • Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers.
  • Ability to communicate effectively with patients and families about treatment plans and care transitions.
  • Demonstrated proficiency with technology including computers, electronic records, and common office equipment.
  • Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes.
