MISSION

SUMMARY

Assists individuals served within the program obtain and maintain employment that is consistent with
their employment goals. Works closely with a variety of staff at LES, as well as community employers, to
help identify and provide the necessary training and supports needed for the individuals' success. Builds
and maintains employment site relationships and serves as a professional role model for interactions
between individuals and other members of the community. Engages in effective data documentation, as needed.
Communicates information to other staff, individuals served, families/guardians and service
providers, as required and in a timely fashion. May work nonstandard work hours, such as early
mornings, evenings and/or weekends and holidays, as needed. Complies with all principles of Human
Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all
other applicable funding regulations as adhered to by Lifeworks.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be
assigned.

Works closely with the job developers to help identify qualified candidates for various companies and
positions identified by the developers.
Acts as a liaison between the job developers, staff, employers, the individuals, families and residences.
Represents Lifeworks in a professional, productive manner.

Communicates with the Career Planner to identify job ready candidates.
Assists in the career planning processes, including updating Career Plans, as needed.
Assists individuals in preparing for the workforce through education, training, and job readiness preparation.
Coordinates with staff with the development of curriculum that helps individuals meet the Action Steps noted in their Career Plans.
Works with the Job Developers to prepare individuals for the interview and hiring process including conducting mock
interviews, as needed.

Supports individuals in overcoming barriers to employment, when possible, and in advancing in their jobs and careers. Assists with onboarding individuals at community work sites, as
needed.

Performs job analysis to determine the needs of the employer and how best the needs can be met by the individual, as needed.
Helps create employment information binders. Teaches and assists staff on how to prepare task analysis.
Teaches and supports staff in acclimating to job sites and assists in troubleshooting concerns.
Provides job coaching supports at work employment sites as needed and assists individuals in maintaining employment, as needed.

Provides any needed interventions/supports by returning phone calls/emails and responding to situations in a timely manner. Supports individuals in advocating for their rights, solving problems, resolving conflicts, and identifying resources. Is involved in the MRC contract process.

Supports employers with respect to information regarding the Americans with Disabilities Act (ADA), obtaining information on tax credits and other employer benefits, and specific supports/accommodations required by individuals, as needed.

Assists individuals in understanding how their state benefits (e.g., SSI, Medicaid, etc.) may be affected by
employment in order for individuals to make good decisions about employment opportunities. Refers clients to benefits counseling, as needed.

Builds and maintains employment site relationships through spontaneous visits, as well as via telephone
and email.
Serves as a professional role model for interactions between individuals and other members of the community.
Communicates information to other staff, individuals served, families/guardians and service providers, as required and in a timely fashion..
Conducts him/herself with the understanding that he/she is a representative of LES and carries the reputation of the agency with them during their interactions with others.
Has ability to work nonstandard work hours, such as early mornings, evenings and/or weekends and holidays, as needed. Attends staff and required meetings as scheduled.

Other duties and/or assignments to work/program locations based on the needs of the company.

SUPERVISORY RESPONSIBILITIES

Directly supervise up to three employees. Carries out supervisory responsibilities in accordance with the
organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training
employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining
employees; addressing complaints and resolving problems.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Bachelor's degree from four-year college or university, and at least one year of related experience; Sales
experience preferred; Ability to work as an effective team player is essential; Ability to learn various
computer programs, including iCentrix, HCSIS and Vertex, required

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and
procedure manuals. Ability to write and proofread routine reports and correspondence. Ability to speak
and present effectively before individuals or employees of an organization.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common
fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or
diagram form. Ability to deal with problems involving several concrete variables in standardized
situations.

VEHICLE/TRAVEL

Requires a legally insured and inspected automobile for use to be present at work and available for use
during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.

CERTIFICATES, LICENSES, REGISTRATIONS

Valid and current driver's license from the state of residency, CPR, First Aid and Medication
Administration Certifications.
