Job Description
A well-established hospice organization is seeking a full-time RN Case Manager to provide compassionate, patient-centered care in a community-based setting. This role is ideal for a nurse who values meaningful work, interdisciplinary collaboration, and balanced caseloads.
The RN Case Manager is responsible for coordinating and delivering hospice care services to assigned patients while supporting families through the end-of-life journey. This role works closely with an interdisciplinary team to ensure quality, compliance, and continuity of care.

What You'll Do:
- Manage 10-14 patients, ensuring they receive appropriate care.
- Complete assessments, create care plans, and document care.
- Collaborate with doctors, social workers, and other team members.
- Provide support and education to families.
- Enjoy flexible scheduling and work-life balance (avoid burnout!).
We're Looking For:
- Idaho RN License.
- 2+ years of experience (pref. hospice).
- Strong communication and teamwork skills.
- Valid driver's license & reliable transportation.

Certifications, Licenses and Registrations
- Registered Nurse licensed in the state of employment.
- Minimum of one (1) year nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years is preferred.
- Certification in Hospice and Palliative Nursing a plus but not required.
- Current CPR certification.
- Must have a valid driver's license and auto liability insurance.
