Job Title: Senior Primary Therapist

Department: Behavioral Health Therapy - ACT Program/Team

Location: Community Based Work - Home Office: Evelyn Brandon Mental Health Center - 81 Lake Ave, Rochester, NY 14608

Hours Per Week: 40

Schedule: Monday - Friday, 8:00 AM - 4:30 PM, On call requirement: Rotating- each team member is on call about every 5 - 6 weeks for 1 week at a time. On call requires being available by phone to respond to patient crises. There is hourly compensation for the after-hours responsibility

ACT Team is a small and cohesive team providing highly needed services to vulnerable people in the community. Ability to be creative in use of treatment models.

SUMMARY:

As a Senior Primary Therapist, your excellent communication and evaluation abilities greatly assist care providers in the assessment and treatment planning for patients with mental illness or those struggling with substance abuse. The primary therapist provides mental health diagnostic assessment, treatment planning, and mental health therapies for persons suffering from mental illness/substance abuse. Health Reach Only:The therapist will work within the Health Reach Health Care for the Homeless Program which incorporates both a central clinical site as well as outreach within the community. Treatment is provided in the context of a multidisciplinary clinical team, supervised by a licensed clinician and program director. Behavioral Health Only: Treatment is provided in the context of a clinical team, supervised by the staff psychiatrist. RESPONSIBILITIES

  • Provides initial and ongoing assessment, counseling and clinical intervention to a designated caseload.
  • Provides individual, group, and family psychotherapy.
  • Participates functionally as a member of a multidisciplinary team in coordination and delivery of patient care, and communicates with relevant internal and external care providers.
  • Participates in and incorporates professional supervision and ongoing education toward quality care
  • Utilizes efficient computer skills to complete all required documentation in a timely and comprehensive manner.
  • Maintains a full caseload, meeting productivity requirements and seeks to keep the incidence of failed appointments to a minimum..
  • Must maintain licensure in a timely manner.
  • Home-Based Crisis Intervention (Unity System): On-Call responsibilities 1 week per month, off-site and in-home service delivery to clients and families, and the same criteria as ACT team as far as driver’s license, etc.
  • Health Reach Only: Will be an active member of the Health Reach Continuous Quality Improvement Committee and will participate in other program and community wide activities.
  • ACT Program: Primary resource for team on integrating family goals and services with the tasks of all team members.
  • ACT Program: Provides individual and group family psycho-education.

REQUIRED QUALIFICATIONS:

  • Master's Degree or Phd in Human Services field required
  • LCSW, LMSW, LMHC, LCAT, LMFT or earned doctorate in human services field required. Limited permit or license required within 6 months of hire

PREFERRED QUALIFICATIONS:

  • Two years of mental health experience preferred
  • Experience in community based work

PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

PAY RANGE: $66,000.00 - $86,000.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
